摘 要:目的探讨育龄期女性阴道微生态失调与高危型人乳头状瘤病毒(HPV)感染之间的关系。方法收集2019年1~12月在兰州大学第一医院和静宁县中医院行HPV及阴道分泌物检测的育龄期女性432例为研究对象。根据HPV及阴道分泌物检测结果将其分为HPV感染高危组(122例)、HPV感染低危组(163例)及HPV感染阴性组(147例),比较三组受检者的阴道微生态失调情况。结果HPV感染高危组女性阴道分泌物中的线索细胞检出率和白细胞酯酶(LE)阳性检出率明显高于HPV感染阴性组和感染低危组,差异均有统计学意义(P<0.05),而HPV感染阴性组和感染低危组间比较差异均无统计学意义(P>0.05);HPV感染高危组女性阴道pH≤4.5的阳性率明显高于HPV感染阴性组和感染低危组高,差异有统计学意义(P<0.05),而HPV感染阴性组与HPV感染低危组女性阴道分泌物中的pH≤4.5的阳性率比较差异无统计学意义(P>0.05);三组女性阴道分泌物中的滴虫、霉菌、乳杆菌、H_(2)O_(2)阳性率和NAG阳性率比较差异均无统计学意义(P>0.05)。结论阴道微生态失调与高危型HPV感染关系密切,其中阴道pH升高、线索细胞阳性、LE阳性可增加高危型HPV感染的风险;滴虫、霉菌、H_(2)O_(2)、NAG与高危型HPV感染的发生不相关。Objective To explore the relationship between vaginal micro-ecological disorder and high-risk human papillomavirus(HPV)infection in women of reproductive age.Methods A total of 432 patients of reproductive age who underwent HPV and vaginal secretions detection in the First Hospital of Lanzhou University and Jingning Hosptial of Traditional Chinese Medicine from January 2019 to December 2019 were enrolled as the research subjects.According to the test results of HPV and vaginal secretions,the subjects were divided into high-risk HPV group(n=122),low-risk HPV group(n=163),and HPV-negative group(n=147).Meanwhile,vaginal micro-ecological disorder of each group was compared.Results The detection rate of clue cells and the positive rate of leucocyte esterase(LE)in the vaginal secretions of women in the high-risk HPV group was higher than that in the HPV-negative group and the low-risk HPV group(P<0.05),while they showed no significant difference between the HPV-negative group and the low-risk HPV group(P>0.05).The positive rate of vaginal p H≤4.5 in the high-risk HPV group was significantly higher than that in the HPV-negative group and the low-risk HPV group(P<0.05),but it showed no significant difference between the HPV-negative group and the low-risk HPV group(P>0.05).There was no significant difference in the positive rates of Trichomonas,Mold,Lactobacillus,H_(2)O_(2),and NAG among the three groups(P>0.05).Conclusion Vaginal microdysbiosis is associated with high-risk group of HPV.The increase of vaginal pH,clue cell-positive,and LE-positive can increase the high-risk HPV infection.However Trichomoniasis,Molds,H_(2)O_(2),and NAG were not related to the occurrence of high-risk HPV infection.
