基于“陆氏针灸”理论温针疗法治疗椎动脉型颈椎病的临床研究

摘　　要：目的:观察“陆氏针灸”温针疗法治疗椎动脉型颈椎病的临床疗效。方法:选取2019年1月—2019年12月在上海市长宁区虹桥街道社区卫生服务中心及下属三个卫生服务站就诊的中老年椎动脉型颈椎病患者60例,随机分为治疗组及对照组,每组30例。治疗组取颈椎局部颈夹脊穴、风池穴、百会穴,在百会穴加温艾灸;对照组取穴、针灸方法、疗程和治疗组均相同,百会穴不进行艾灸,用红外线TDP直接照射加热。留针20 min,隔日针灸1次,10次为1个疗程。治疗2个疗程。采用颈性眩晕症状与功能评估(evaluation scale for cervicalvertigo,ESOV)量表评定椎动脉型颈椎病症状及体征,观察疗效。结果:治疗组总有效率是93.10%(27/29);高于对照组的86.21%(25/29,P<0.05)。两组患者治疗后ESOV量表指标与治疗前相比较均有不同程度的提高(P<0.05),且治疗组优于对照组(P<0.05)。结论:“陆氏针灸”温针疗法治疗椎动脉型颈椎病具有疗效显著,取穴精简,操作简便,安全无副作用,依从性强等特点,可在临床上推广应用。Objective:To observe the clinical effect of warm needling therapy of Lu's acupuncture and moxibustion on cervical spondylosis of vertebral artery type.Methods:From January to December 2019,60 middle-aged and elderly patients with cervical spondylosis of vertebral artery type were selected from Hongqiao Community Health Service Center and its three subordinate health service stations of Changning District in Shanghai,and randomly divided into a treatment group and a control group with 30 cases in each group.In the treatment group,the local cervical Jiaji acupoint,Fengchi acupoint and Baihui acupoint were taken,and moxibustion was added at Baihui acupoint;in the control group,acupoint selection,acupuncture method,course of treatment were the same of the treatment group.Baihui acupoint was not moxibustion,and was directly irradiated and heated by infrared TDP.The needles were kept for 20 minutes,and acupuncture and moxibustion was every other day,10 times as a course.The treatment was for 2 courses.The evaluation scale for cervicalvertigo(ESOV)scale was used to assess the symptoms and signs of cervical spondylosis of vertebral artery type to observe the curative effect.Results:The overall effective rate in the treatment group was 93.10%(27/29);it was higher than that 86.21%in the control group(25/29,P<0.05).The ESOV scale indicators of the two groups of patients were improved to varying degrees after treatment compared with those before treatment(P<0.05),and the treatment group was better than the control group(P<0.05).Conclusion:Warm needling therapy of“Lu’s acupuncture and moxibustion”has remarkable curative effect in the treatment of cervical spondylosis of vertebral artery type,simple acupoint selection,simple operation,safety and no side effects,strong compliance and other characteristics,which can be popularized and applied in clinical practice.
