摘 要:目的分析微创弹性髓内钉技术(ESIN)与石膏固定治疗小儿股骨干骨折的效果及其对患者膝关节功能的影响。方法选取本院收治的股骨干骨折小儿76例,将其分为石膏固定组36例及微创组40例。石膏固定组牵引后给予石膏固定,微创组给予微创弹性髓内钉治疗。观察并记录两组患者术中情况(切口长度、出血量及手术时间)、视觉模拟疼痛评分(VAS)、膝关节功能、恢复情况、并发症发生情况等。结果微创组手术时间长于石膏固定组(P<0.05);微创组术后VAS评分、术后恢复时间、并发症总发生率均显著低于石膏固定组(P<0.05);微创组膝关节功能优良率高于石膏固定组(χ^(2)=5.28,P<0.05)。结论微创弹性髓内钉较石膏固定更能有效改善小儿股骨干骨折治疗效果及逛街功能,减少其术后疼痛及并发症的发生,有助于患者术后恢复。Aim To explore and analyze the effect of minimally invasive elastic intramedullary nail technology(ESIN)and plaster fixation in the treatment of femoral shaft fractures in children and its influence on knee joint function.Methods 76 children with femoral shaft fracture admitted to our hospital were selected and randomly divided into plaster fixed group(n=36)and minimally invasive group(n=40).The plaster fixed group was given plaster fixation after traction,and the minimally invasive group was treated with minimally invasive elastic intramedullary nail.The general data intraoperative conditions(incision length,blood loss and operation time),visual analogue scale(VAS),knee joint function,recovery,and complications were observed and recorded.Results The operation time of the minimally invasive group was significantly longer than that of the plaster fixed group(P<0.05);the postoperative VAS score,the postoperative recovery time and the total incidence of complications of the minimally invasive group were significantly lower than those of the plaster fixed group(P<0.05);the excellent and good rate of knee joint function in the minimally invasive group was significantly higher than that in the plaster fixed group(χ^(2)=5.28,P<0.05).Conclusion Compared with plaster fixation,minimally invasive elastic intramedullary nail can effectively improve the treatment effect and shopping function of femoral shaft fracture in children,reduce the incidence of postoperative pain and complications,and help patients recover after operation.
