山东省某地健康人群MCR-1阳性大肠埃希菌和阴性大肠埃希菌耐药性比较研究

摘  要:目的对携带多粘菌素耐药基因(mcr-1)大肠埃希菌和未携带mcr-1大肠埃希菌耐药情况进行比较,为进一步探索携带mcr-1对耐药性的影响提供线索。方法对2017年山东省某地健康居民中收集的粪便样本分离到的156株mcr-1阳性大肠埃希菌和184株mcr-1阴性大肠埃希菌,通过PCR检测mcr-1,采用琼脂稀释法对菌株进行12种抗菌药物敏感性实验。结果 mcr-1阳性大肠埃希菌对多粘菌素均耐药,耐药率为100.00%,mcr-1阴性大肠埃希菌对多粘菌素均敏感;mcr-1阳性大肠埃希菌耐药率均高于阴性菌,其中对多西环素、复方新诺明、氨苄西林的耐药率为88.46%、78.85%、68.59%,明显高于mcr-1阴性菌的52.17%、34.24%、47.28%;mcr-1阳性大肠埃希菌中多重耐药菌所占比例为87.18%,高于阴性菌的48.37%,(P<0.05);mcr-1阳性大肠埃希菌对每种抗菌药物MIC值的几何均数均高于阴性菌(P<0.05);所有阳性菌和阴性菌对美罗培南均敏感。结论 mcr-1阳性菌的耐药情况比阴性菌严重;携带mcr-1与多粘菌素耐药密切相关,且mcr-1可能合并其他耐药基因,增强细菌的耐药性。Objective To compare the drug resistance of mobile colistin resistance gene(mcr-1) positive Escherichia coli and mcr-1 negative Escherichia coli,so as to provide clues for further exploration of the effect of carrying mcr-1 on drug resistance. Methods The 156 strains of mcr-1 positive Escherichia coli and 184 strains of mcr-1 negative Escherichia coli were isolated from stool samples collected from a healthy population in Shandong province in 2017.The mcr-1 was detected by PCR,and 12 antimicrobial susceptibility tests were conducted by AGAR dilution method. Results All mcr-1 positive Escherichia coli were resistant to polymyxin, with a resistance rate of 100.00% and all mcr-1 negative Escherichia coli were susceptible to polymyxin.The resistance rates of mcr-1 positive Escherichia coli were all higher than those of negative bacteria, with 88.46%,78.85% and 68.59% for doxycycline, cotrimoxazole and ampicillin, which were significantly higher than the 52.17%,34.24% and 47.28% for mcr-1 negative bacteria.The proportion of multi-drug resistant bacteria among mcr-1 positive Escherichia coli was 87.18%,higher than the 48.37% of negative bacteria(P<0.05).The geometric mean MIC values of mcr-1 positive Escherichia coli were higher than those of negative Escherichia coli(P<0.05).All positive and negative bacteria were sensitive to meropenem. Conclusion The drug resistance of mcr-1 positive bacteria is more serious than that of negative bacteria.Carriage of the mcr-1 is closely related to polymyxin resistance, and the mcr-1 may combine with other resistance genes to enhance bacterial resistance.
