摘  要:目的通过耐药监测了解北京市怀柔区肺结核患者耐药状况,为结核病防治提供参考依据。方法收集怀柔区2013年1月-2019年12月新登记活动肺结核患者中获得的199例菌阳肺结核患者的临床分离株,进行菌型鉴定,对分离菌株进行药物敏感性试验。对各种药物的初始耐药率、获得性耐药率、耐多药率和耐药顺位的差异进行分析。结果分离的199株菌株经菌型鉴定,共有结核分枝杆菌185株,总体耐药率为24.32%(45/185),总耐多药率为4.86%(9/185)。获得性耐药率(42.42%,14/33)高于初始耐药率(20.39%,31/152),差异有统计学意义(χ^(2)=7.148,P<0.05)。8种药物耐药率顺位由高到低依次为SM(14.05%,26/185),INH(11.89%,22/185),RFP(8.11%,15/185),OFX(7.57%,14/185),EMB(4.32%,8/185),PTO(1.62%,3/185),KM和CPM(均为1.08%,2/185)。结论怀柔区结核分枝杆菌耐药情况与全国一些地区基本相同,形势依然比较严峻,仍需继续加强对耐药结核病的控制工作。Objective Drug resistance surveillance is used to understand the drug resistance status of tuberculosis patients in Huairou district,Beijing,and to provide a reference basis for the prevention and treatment of tuberculosis in the region.Methods The clinical isolates of 199 positive pulmonary tuberculosis patients obtained from newly registered active tuberculosis patients in Huairou district from January 2013 to December 2019 were collected,and the bacterial types were identified,and drug susceptibility test was performed on the isolated strains.The differences of the initial drug resistance rate,acquired drug resistance rate,multidrug resistance rate,and resistance order of various drugs were analyzed.Results The 199 isolates strains were identified by type identification,and there were 185 Mycobacterium tuberculosis strains.The overall drug resistance rate was 24.32%(45/185),and the total multidrug resistance rate was 4.86%(9/185).The acquired drug resistance rate of 42.42%(14/33)was higher than the initial resistance rate of 20.39%(31/152),with a statistically significant difference(χ^(2)=7.148,P<0.05).The 8 drug resistance rates in descending order were SM(14.05%,26/185),INH(11.89%,22/185),RFP(8.11%,15/185),OFX(7.57%,14/185),EMB(4.32%,8/185),PTO(1.62%,3/185),KM and CPM(both 1.08%,2/185).Conclusions The resistance of Mycobacterium tuberculosis in Huairou district of Beijing is basically the same as that in some parts of the country.The situation is still severe,and there is still a need to continue to strengthen the control of drug-resistant tuberculosis.
