摘  要:目的评估区域折射型多焦点人工晶状体SBL-3(Lenstec,Barbados)较单焦点非球面人工晶状体Adapt AO(Bausch&Lomb,USA)植入术后患者中、长期临床疗效。方法纳入2019年8月至2020年6月安徽省立医院年龄相关性白内障患者43例(86只眼),均行超声乳化白内障摘除及囊袋内人工晶状体植入术。依据植入人工晶状体分为两组,其中植入SBL-3组25例(50只眼),Adapt AO组18例(36只眼)。比较两组间术后3个月以上的眼科检查包括裸眼远视力、裸眼中视力、裸眼近视力、离焦曲线、调制传递函数(MTF)、Strehl比(SR)、波前像差,并对所有患者进行VF-14(中文译版)评分。结果SBL-3组与AdaptAO组术后裸眼远视力差异无统计学意义(P=0.4707)。SBL-3组术后裸眼中、近视力明显优于Adapt AO组(P=0.0009、P<0.0001)。两组离焦曲线从-1.5 D到4.0 D,差异有明显统计学意义,且SBL-3组视力较好。在客观光学指标方面,全眼高阶像差、彗差和三叶草差的均方根值在两组之间有统计学差异,SBL-3组明显高于Adapt AO组(P<0.0001),两组间球差无明显统计学差异。全空间频率的MTF值、SR值在组间差异有统计学意义(P<0.0001)。在SBL-3组中,瞳孔直径与MTF平均高度(R=-0.6069,P<0.0001)和SR(R=-0.7138,P<0.0001)呈负相关,而与总高阶像差的均方根值呈正相关(R=0.6993,P<0.0001)。两组间VF-14近距离项目评分差异有统计学意义(P<0.0001)。结论区域折射型多焦点人工晶状体SBL-3植入术后能为患者提供较满意的远、中、近视力和视觉质量,一定程度上提高了患者的生存质量。Objective To evaluate the medium and longterm clinicalcurativeeffects of a rotationally asymmetric multifocal intraocular lens(MIOL)SBL-3(Lenstec,Barbados)and a monofocalasphericintraocular lens Adapt AO(Bausch&Lomb,USA).Methods Forty-three age-related cataract patients(86 eyes)in Anhui Provincial Hospital from August 2019 to June 2020were enrolled.All of them underwent phacoemulsification cataract surgery and in-the-bag IOL implantation.According to the type of intraocular lens implanted,they were divided into two groups,including 25 patients(50 eyes)in the SBL-3 group and 18 patients(36 eyes)in the Adapt AO group.Ophthalmological examinations includeduncorrected distance visual acuity(UDVA),uncorrected intermediate visual acuity(UIVA),uncorrected near visual acuity(UNVA),defocus curve,modulation transfer function(MTF),Strehl ratio(SR)and wavefront aberrations at more than 3 months postoperatively.The visual function index-14 were assessed in all patients.Results There was no statistically significant difference between groups in postoperative UDVA(P=0.4707).Postoperative UIVA and UNVA were significantly better for the SBL-3 group than for the Adapt AO group(P=0.0009、P<0.0001).Statistically significant differences were revealed in defocus levels from-1.5 D to 4.0 D with better visual acuities for the SBL-3 group.For optical quality outcomes,statistically significant differences between groups were observed in rootmeansquare(RMS)of total high-order aberrations,coma andtrefoil,presenting significantly higher values of these parameters in the eyesof the SBL-3 group(P<0.0001).There was no significant difference in spherical aberration(SA)between the two groups.Statistically significant differences were revealed in the SR valuesandMTF values at allspatial frequencies between groups(P<0.0001).In the SBL-3 group,Pupil size were negatively correlated with MTF average height values(R=-0.6069,P<0.0001)and SR values(R=-0.7138,P<0.0001),while pupil size were positively correlated with root mean square(RMS)of total high-or
