摘  要:目的采用双通道视觉质量分析系统(OQAS)分析研究糖尿病患者泪膜及视觉质量变化情况。方法前瞻性病例研究。收集2020年10月至2021年4月在安徽医科大学第一附属医院就诊2型糖尿病患者组31例(38只眼),按照糖尿病视网膜病变(DR)分期分为无DR、非增生期DR及增生期DR3组,利用双通道视觉质量分析系统Ⅱ(OQASII)评估入组患者视觉质量及泪膜稳定性,并结合眼表临床检查、泪膜稳定性试验评价泪膜及眼表功能。对所有入选对象进行眼表疾病指数(OSDI)问卷调查,记录糖尿病病程、糖化血红蛋白(HbA1c)水平。结果客观散射指数(OSI)与眼底视网膜病变分期呈正相关(r=0.337,P<0.05),与糖基化血红蛋白正相关(r=0.353,P<0.05);平均客观散射指数(mean-OSI)与眼底视网膜病变分期呈正相关(r=0.267,P<0.05),泪膜破裂时间(TBUT)与糖基化血红蛋白(HbA1c)呈正相关(r=0.394,P<0.05),OSI与平均OSI呈显著相关(r=0.783,P<0.01),结论DR严重程度及患者血糖控制情况与视觉质量及泪膜稳定性密切相关。Objective To evaluate the changes of ocular surface and tear film optical quality in type 2 diabetic patients by using the optical quality analysis system(OQASII).Methods Thirty-eight eyes of 31patients with diagnosis type 2 diabetes was recruited in a prospective study,who were admitted to the First Affiliated Hospital of Anhui Medical University from October 2020 to April 2021.The tear film optical quality of the enrolled patients was evaluated using the optical quality analysis system(OQASII).The tear film and ocular surface function were evaluated by ocular surface clinical examination and tear film stability test.Ocular surface disease index(OSDI)questionnaire was administer to all subjects.Duration of diabetes,HbA1c level and stage of diabetic retinopathy were recorded.Results The objective scattering index(OSI)was positivelycorrelated with the stage ofdiabetic retinopathy(r=0.337,P<0.05),was associated with glycosylated hemoglobin(r=0.353,P<0.05);Mean objective scattering index(mean-OSI)was positively correlated with the severity of diabeticretinopathy(r=0.267,P<0.05),the tear film rupture time(TBUT)was positively correlated with glycosylated hemoglobin(HbA1c)(r=0.394,P<0.05);There was a significant correlation between OSI and mean-OSI(r=0.783,P<0.01).Conclusions The severity of diabetic retinopathy and blood glucose control in diabetic patients are closely related to visual quality and tear film stability.
