摘  要:目的研究青年男性基层士兵与健康青年男性踝关节软骨厚度值、负荷下踝关节软骨厚度变化的差异。方法以20例青年男性基层士兵为实验组,20例青年男性健康人为对照组,两组年龄无明显差异。利用自制下肢关节负荷装置,对其右踝关节施加负荷,加压前后行3.0 T磁共振扫描采用T1 VIBE 3D FS序列进行右踝关节软骨成像,分别测量加压前后踝关节软骨三个兴趣部位厚度值,并计算负荷下踝关节软骨厚度压缩比率。结果基层士兵和对照组的踝关节软骨厚度值分别为:外侧(2.76±0.36)mm和(2.30±0.30)mm(P<0.001);中间(2.72±0.32)mm和(2.23±0.26)mm(P<0.001);内侧(2.79±0.36)mm和(2.31±0.28)mm(P<0.001)。基层士兵踝关节软骨厚度在加压前后分别为:外侧(2.76±0.36)mm和(2.45±0.36)mm(P<0.001);中间(2.72±0.32)mm和(2.46±0.29)mm(P<0.001);内侧(2.79±0.36)mm和(2.52±0.36)mm(P<0.001)。对照组踝关节软骨厚度在加压前后分别为外侧(2.30±0.30)mm和(2.20±0.29)mm(P<0.001);中间(2.23±0.26)mm和(2.13±0.25)mm(P<0.001);内侧(2.31±0.28)mm和(2.22±0.26)mm(P<0.001)。基层士兵和对照组负荷下踝关节软骨厚度压缩率分别为:外侧(11.22±4.81)%和(4.15±1.43)%(P<0.001);中间(9.65±2.65)%和(4.42±1.48)%(P<0.001);内侧(9.95±3.95)%和(4.13±1.35)%(P<0.001);基层士兵负荷下踝关节软骨厚度压缩率与每周训练总时长具有相关性(P<0.001)。结论基层士兵踝关节软骨厚度值高于对照组;负荷下踝关节软骨厚度值减小;基层士兵负荷下踝关节软骨厚度压缩率高于对照组,并且每周军事训练时长越久、压缩率相对越高。Objective To study the difference of ankle cartilage thickness and its change under load between young male grass-roots soldiers and healthy young male control group.Methods 20 young male grass-roots soldiers and 20 age-matched healthy men were enrolled in this study.3T MRI was performed with 3-D fat-saturated T1-weighted sequence before and after compression was applied to the right ankle using a lower extremity joint load device.The cartilage thickness of three parts of the ankle was measured and the compression ratio under load was calculated.Results The cartilage in the lateral,central,and medial ankle was significantly thicker(P<0.001)in the soldiers(2.76±0.36)mm,(2.72±0.32)mm,(2.79±0.36)mm than the control group(2.30±0.30)mm,(2.23±0.26)mm,(2.31±0.28)mm.The lateral,central,and medial cartilage was significantly thicker(P<0.001)before(2.76±0.36)mm,(2.72±0.32)mm,(2.79±0.36)mm than that after(2.45±0.36)mm,(2.46±0.29)mm,(2.52±0.36)mm compression in the soldiers.The lateral,central,and medial cartilage was also significantly thicker(P<0.001)before(2.30±0.30)mm,(2.23±0.26)mm,(2.31±0.28)mm than that after(2.20±0.29)mm,(2.13±0.25)mm,(2.22±0.26)mm compression in the control group.The compression ratios of lateral(11.22±4.81)%,central(9.65±2.65)%,and medial(9.95±3.95)%cartilage thickness under load in the soldiers were significantly higher(P<0.001)than that of the control group(4.15±1.43)%,(4.42±1.48)%,(4.13±1.35)%.There was correlation between the compression ratios of cartilage thickness under load and the total training time per week of soldiers(P<0.001).Conclusion The thickness of ankle cartilage in grass-roots soldiers was higher than that of control group.The cartilage thickness decreased under load with higher compression ratio in soldiers and longer military training.
