高频电针对腹泻型肠易激综合征大鼠血清VIP、MTL表达及内脏敏感性的影响

摘　　要：目的观察高频电针对腹泻型肠易激综合征(IBS)大鼠血清血管活性肠肽(VIP)、胃动素(MTL)表达水平及内脏敏感性的影响。方法SPF级大鼠40只,随机分为对照组、模型组、西药组、电针组,每组10只,除对照组外均采用束缚应激刺激加番泻叶煎剂(0.3 g/mL)灌胃(20 mL/kg)2周造模。电针组每天治疗20 min,隔1 d治疗1次,治疗2周;西药组给予25 mg/kg匹维溴铵灌胃(质量浓度2.5 mg/mL,灌胃体积10 m L/kg),每天给药1次,连续给药2周;模型组和对照组给予等量的生理盐水。治疗结束后称量大鼠体质量,并进行腹部抬起、背部拱起的腹部回撤反射(AWR)容量阈值检测。酶联免疫吸附法(ELISA)检测大鼠血清VIP、MTL表达水平,苏木精-伊红(HE)染色观察各组大鼠结肠组织病理学变化。结果对照组大鼠精神状态、大小便及饮食均正常,肠腺体中杯状细胞大小基本整齐,无充血现象;模型组大鼠精神状态欠佳、体质量降低、粪便稀黏肠腺体形态不完整,黏液/上皮细胞脱落,隐窝消融,充血明显;西药组与电针组大鼠活动量增加,精神恢复,饮食饮水增多,大便正常,肠腺体隐窝和绒毛消融现象减少,无充血现象。与对照组相比,模型组大鼠体质量、腹部抬起、背部拱起的AWR的容量阈值降低,血清VIP、MTL水平升高(P<0.05);与模型组相比,西药组、电针组大鼠体质量、腹部抬起、背部拱起的AWR的容量阈值升高,血清VIP、MTL水平降低(P<0.05);西药组与电针组上述指标比较差异无统计学意义(P>0.05)。结论高频电针对腹泻型IBS大鼠具有保护作用,可能与抑制VIP、MTL表达,提高大鼠AWR容量阈值有关,可作为腹泻型IBS潜在的治疗方法。Objective:To investigate the effects of high-frequency electroacupuncture on the expression levels of vasoactive intestinal peptide(VIP)and motilin(MTL)in serum and visceral sensitivity in rats with diarrhea irritable bowel syndrome(IBS).Methods:A total of 40 SPF rats were random divided into the control group,model group,western medicine(pinaverium bromide)group and electroacupuncture group,10 rats in each group.Except for the control group,the rats were treated with restraint stress stimulation and given senna leaf decoction(0.3 g/mL)by irrigation(20 mL/kg)for 2 weeks to establishing the model.The electroacupuncture group was treated for 20 minutes every other day for 2 weeks;the western medicine group was treated with 25 mg/kg pinaverium bromide by gavage(concentration 2.5 mg/mL,gavage volume 10 m L/kg),once a day for 2 weeks;the model group and the control group were given the same amount of normal saline.The rats were weighed after treatment,and the abdominal withdrawal reflex(AWR)abdominal lifting and back arching capacity thresholds were detected.The expression levels of VIP and MTL in serum were detected by enzyme-linked immunosorbent assay(ELISA),and the pathological changes of colon were observed by hematoxylin eosin(HE)staining.Results:The rats in the control group had normal mental state,defecation and diet,and the goblet cells in the intestinal glands were basically uniform in size,without congestion;the rats in the model group had poor mental state,weight loss,loose feces,incomplete intestinal gland morphology,mucus/epithelial cells shedding,crypt melting,and obvious congestion;the rats in the western medicine group and electroacupuncture group had increased activity,mental recovery,diet and drinking water,normal stool,and reduced intestinal gland crypt and villi ablation phenomenon,with no congestion phenomenon.Compared with the control group,the body weight of rats,capacity threshold of AWR with abdomen raised and back arched in the model group decreased,while the serum VIP and MTL levels increase
