五风穴联合晕听区针刺与前庭代偿理论探讨

摘　　要：前庭系统是人体重要的平衡系统。前庭功能损伤患者会出现明显的眩晕、眼球震颤、平衡障碍等,然而大部分前庭病变患者在一定时间内眩晕及眼震会逐渐消失,并重新建立肢体平衡,这一主动修复过程称为前庭代偿。外周前庭损伤后中枢的适应性和可塑性是前庭代偿的基础,单侧前庭损伤后双侧前庭神经核、小脑、脊髓、大脑皮层等通过交互纤维逐渐实现静态代偿与动态代偿,但临床中很多患者不能及时完全建立代偿,遗留不同程度的头晕及平衡障碍,影响正常工作与生活。目前促进前庭代偿主要依靠前庭康复训练,且基本以患者居家自行锻炼的方式开展,患者依从性不高,疗效不稳定。近年来研究发现,针灸对于前庭损伤引起的眩晕疾病具有确切的疗效,本文旨在探讨针刺五风穴(双翳风、双风池、风府)及晕听区与前庭代偿之间的密切关联,以期为针刺促进前庭代偿提供理论依据,为前庭病变临床治疗提供可行方法。The vestibular system is an important balance system of human body.Vestibular dysfunction can cause obvious vertigo,nystagmus and imbalance,etc.However,most patients with vestibular lesions will gradually lose their vertigo and nystagmus within a certain period and re-establish limb balance.This process of proactive restoration is called vestibular compensation.After peripheral vestibular dysfunction,the adaptability and plasticity of the central nervous system are the basis of vestibular compensation.After unilateral vestibular dysfunction,bilateral vestibular nucleus,cerebellum,spinal cord,and cerebral cortex gradually realize static and dynamic compensation through interactive fibers.Yet many patients cannot fully establish compensation in time and leave vertigo and balance disorders of varying degrees,which affects normal work and life.In recent years,studies have found that acupuncture has a definite effect on the treatment of vertigo caused by vestibular dysfunction.The purpose of this article is to explore the close relationship between acupuncture at Wufeng Acupoint(Bilateral Yifeng,Bilateral Fengchi,Fengfu)combined with Yunting Zone and vestibular compensation,in order to provide theoretical basis for acupuncture to promote vestibular compensation and provide a feasible method for clinical treatment of vestibular lesions.
