摘 要:目的探究内科胸腔镜联合尿激酶胸腔注射对结核性包裹性胸膜炎的临床疗效。方法选取105例结核性包裹性胸膜炎患者的临床资料进行回顾性分析,根据方案不同分为内科胸腔镜联合尿激酶组(A组)、胸腔置管联合尿激酶组(B组)、胸腔置管组(C组)。比较分析3组患者胸腔积液引流总量、胸腔置管时间、住院时间;比较3组患者出院6个月后的临床疗效。结果住院期间A组患者的胸腔积液引流总量多于B、C组,且B组多于C组,差异均有统计学意义(P<0.05);A组患者的胸腔置管时间、住院时间短于B、C组,且B组短于C组,差异均有统计学意义(P<0.05)。治疗6个月后A组总有效率高于B、C组,且B组高于C组,差异均有统计学意义(P<0.05)。结论内科胸腔镜联合尿激酶胸腔注射能够有效提高结核性包裹性胸膜炎患者的临床疗效,值得临床推广使用。Objective To investigate the clinical effect of medical thoracoscopy combined with intrapleural injection of urokinase in the treatment of encapsulated tuberculous pleurisy.Methods Retrospective analysis was conducted on the clinical data of 105 patients with encapsulated tuberculous pleurisy.According to different treatment regiments,the patients were divided into medical thoracoscopy combined urokinase group(Group A),center venous intrapleural catheterization drainage combined urokinase group(group B),and center venous intrapleural catheterization drainage group(group C).The total amount of pleural effusion,duration of intrapleural catheterization and length of stay were compared and analyzed among the 3 groups.The results of clinical treatment after 6 months were compared among 3 groups.Results During hospitalization,the total amount of pleural effusion was higher in group A than in group B and C,and the total amount of pleural effusion in group B was higher than that in group C,with statistically significant differences(P<0.05).The length of intrapleural catheterization and hospitalization of group A were shorter than those of groups B and C,and the duration of intrapleural catheterization and hospitalization of group B were shorter than those of group C,with statistically significant differences(P<0.05).After 6 months of treatment,the total effective rate of group A was higher than that of group B and C,and the total effective rate of group B was higher than that of group C,with statistically significant differences(P<0.05).Conclusion Medical thoracoscopy combined with intrapleural injection of urokinase can effectively improve the clinical treatment effect of patients with encapsulated tuberculous pleurisy,which is worth popularizing in clinical practice.
