摘　　要：目的观察充血性心力衰竭(CHF)患者血清心肌营养素1(CT-1)、细胞间黏附分子1(ICAM-1)及N-末端脑钠肽原(NT-proBNP)水平变化,并分析其与心功能关系。方法选取山东省立第三医院2018年4月至2019年8月112例CHF患者为观察组,按照美国纽约心脏病学会(NYHA)心功能分级分为三个亚组(Ⅱ级组、Ⅲ级组、Ⅳ组),并取同期50例体检健康者作为对照组,检测各组血清CT-1、ICAM-1、NT-proBNP水平,并利用超声仪测定心功能指标[左心室射血分数(LVEF)、左心室收缩末内径(LVESD)与左心室舒张末内径(LVEDD)],进行相关性分析。结果观察组血清CT-1、ICAM-1、NT-proBNP水平显著高于对照组(t=18.919、14.795、127.439,P<0.05);CHF患者不同心功能分级亚组血清CT-1、ICAM-1、NT-proBNP水平比较差异均有统计学意义(F=102.424、11.194、127.439,P<0.05),Ⅳ级组血清CT-1、ICAM-1、NT-proBNP水平明显高于Ⅱ级组与Ⅲ级组(P<0.05),Ⅲ级组明显高于Ⅱ级组(P<0.05)。CHF患者不同心功能分级亚组心功能指标比较差异均有统计学意义(F=84.148、30.930、21.158,P<0.05),Ⅳ级组患者LVEF明显低于Ⅱ级组与Ⅲ级组(P<0.05),LVESD、LVEDD明显大于Ⅱ级组与Ⅲ级组(P<0.05),Ⅲ级组LVEF明显低于Ⅱ级组(P<0.05),LVESD、LVEDD明显大于Ⅱ级组(P<0.05)。Pearson分析显示血清CT-1、ICAM-1、NT-proBNP与LVEF呈负相关(r=-0.978、-0.989、-0.991,P<0.05)。结论CHF患者血清CT-1、ICAM-1、NT-proBNP水平有异常升高改变,且心功能损伤越严重,升高程度越大,与LVEF呈显著负相关。Objective To observe serum levels of cardiotrophin 1(CT-1),intercellular adhesion molecule 1(ICAM-1)and N-terminal pro-brain natriuretic peptide(NT-proBNP)in patients with congestive heart failure(CHF),and analyze their relationship with cardiac function.Methods A total of 112 CHF patients admitted to Shandong Provincial Third Hospital between April 2018 and August 2019 were selected as the observation group.Then,they were divided into three subgroups(grade Ⅱ group,grade Ⅲ group,and grade Ⅳ group)according to the New York Heart Association(NYHA)cardiac function classification.Meanwhile,50 healthy subjects were recruited as the control group.Serum CT-1,ICAM-1 and NT-proBNP levels in each group were detected,the cardiac function indexes[left ventricular ejection fraction(LVEF),left ventricular end systolic diameter(LVESD)and left ventricular end diastolic inner diameter(LVEDD)]were measured by ultrasound instrument,and correlation analysis was performed.Results Serum CT-1,ICAM-1,and NT-proBNP levels in the observation group were significantly higher than those in the control group(t=18.919,14.795,127.439;P<0.05).The serum levels of CT-1,ICAM-1 and NT-proBNP in CHF patients with different cardiac function grading subgroups were statistically significant(F=102.424,11.194,127.439;P<0.05).Serum CT-1,ICAM-1,and NT-proBNP levels in the grade Ⅳ group were significantly higher than those in grade Ⅱ and Ⅲ groups(P<0.05),and those in grade Ⅲ group were significantly higher than those in grade Ⅱ group(P<0.05).There were statistically significant differences in the cardiac function indexes of CHF patients in different cardiac function grade subgroups(F=84.148,30.930,21.158;P<0.05).The LVEF of grade Ⅳ group was significantly lower than that of grade Ⅱ group and grade Ⅲ group(P<0.05),LVESD and LVEDD were significantly higher than those of grade Ⅱ group and grade Ⅲ group(P<0.05).LVEF in grade Ⅲ group was significantly lower than that in grade Ⅱ group(P<0.05),and LVESD and LVEDD were significantly hig
