摘  要:目的:分析糖尿病肾脏病(DKD)各期患者血清可溶性肿瘤坏死因子1型受体(sTNFR1)、可溶性肿瘤坏死因子2型受体(sTNFR2)的表达差异及其临床意义,并研究二者与中医内热证之间的相关性。方法:收集209例DKD患者的临床资料及中医证候,检测其血清sTNFR1、sTNFR2等炎症因子的水平,进行统计分析。结果:较健康对照及糖尿病患者,血清sTNFR2水平在DKD早期即出现显著升高,且随着DKD病情加重而逐渐升高,早、中、晚各期之间差异具有统计学意义;在各内热证之中,湿热证患者的血清sTNFR2水平显著高于其他类型内热证患者,且与湿热积分呈明显的正相关关系(r=0.278)。结论:与血清sTNFR1相比,DKD患者肾功能进展可能与血清sTNFR2的关系更为密切,当患者出现湿热证时,血清sTNFR2水平可能更高,提示预后不佳。Objective:To analyze the expression differences of soluble tumor necrosis factor receptor-1(sTNFR1)and soluble tumor necrosis factor receptor-2(sTNFR2)of patients in different stages of diabetic kidney disease(DKD)and investigate their correlation with inner heat syndrome.Methods:The clinical information and traditional Chinese medicine syndrome of 209 DKD patients were collected and their sTNFR1、sTNFR2 were detected and analyzed.Results:Compared with healthy controls and diabetes group,sTNFR2 of patients in the early stage of DKD was significantly higher,and the more severe the disease,the higher the level of sTNFR2.sTNFR2 of patients with dampness heat syndrome was significantly higher than that in patients with other heat syndromes,and there was a significant positive correlation between the dampness heat scores and the level of sTNFR2(r=0.278).Conclusion:Compared with sTNFR1,sTNFR2 tend to be more closely related to the progression of DKD and patients with dampness heat syndrome tend to have higher level of s TNFR2 and a poor prognosis.
