摘  要:目的探讨小剂量红霉素联合布拉氏酵母菌散治疗早产儿喂养不耐受的效果。方法选取漯河市第二人民医院2019年6月至2020年10月收治的86例早产儿为研究对象。按治疗方案将早产儿分为对照组和观察组,各43例。两组均接受基础治疗,对照组患儿接受小剂量红霉素治疗,观察组患儿接受小剂量红霉素联合布拉氏酵母菌散治疗。观察治疗后两组总有效率、不同时段肠道杆菌群(双歧杆菌、乳杆菌)数量变化、生长发育情况及恢复情况(呕吐消失时间、恢复出生体质量时间、达全胃肠喂养时间及住院时间)变化。结果观察组总有效率高于对照组(P<0.05);治疗3 d观察组双歧杆菌数量高于对照组(P<0.05);治疗7、14 d观察组乳杆菌、双歧杆菌数量高于对照组(P<0.05);治疗后观察组身长、体质量大于对照组(P<0.05);观察组住院时间、呕吐消失时间、达全胃肠喂养时间、恢复出生体质量时间均短于对照组(P<0.05);两组均未发生皮疹、发热、心律失常、肝肾功能异常、腹胀、腹泻、呕吐加重、益生菌败血症等不良反应。结论小剂量红霉素联合布拉氏酵母菌散治疗早产儿喂养不耐受的效果显著,能改善胃肠道菌群环境,促进生长发育。Objective To explore the efficacy of low-dose erythromycin combined with Saccharomyces boulardii sachet in the treatment of feeding intolerance in premature infants.Methods A total of 86 premature infants from June 2019 to October 2020 in Luohe Second People’s Hospital were selected.According to the treatment plan,the premature infants were divided into control group and observation group,each with 43 cases.Both groups were given basic treatment.The children in control group were given low-dose erythromycin treatment,and the children in observation group were given low-dose erythromycin with Saccharomyces boulardii sachet.The total effective rate of the two groups after treatment,changes in the number of intestinal bacilli(Bifidobacterium,Lactobacillus)at different times,growth and development and recovery(time to disappear from vomiting,time to restore birth weight,time to reach total gastrointestinal feeding and time in the hospital)changes.Results The total effective rate in observation group was higher than that in control group(P<0.05).The number of Bifidobacterium in observation group was higher than that in control group for 3 days(P<0.05).The number of Lactobacillus and Bifidobacterium in observation group was higher than that in control group at 7 and 14 days of treatment(P<0.05).The length and weight in observation group after treatment were larger than those in control group(P<0.05).The average length of stay in observation group,time to disappear vomiting,time to full gastrointestinal feeding,and time to regain birth weight were shorter than those in control group(P<0.05).There were no adverse reactions such as skin rash,fever,arrhythmia,abnormal liver and kidney function,abdominal distension,diarrhea,aggravated vomiting and probiotic sepsis in both groups.Conclusion Low-dose erythromycin combined with Saccharomyces boulardii sachet has a significant effect on the treatment of feeding intolerance in premature infants.It can improve the environment of the gastrointestinal flora,promote growth and dev
