超声心动图诊断肥厚型心肌病患者的临床应用分析

摘　　要：目的:探讨超声心动图(UCG)、心电图(ECG)诊断肥厚型心肌病(HCM)的价值。方法:回顾分析福建省立医院165例HCM患者临床资料,所有患者均接受ECG、UCG检查并给出诊断结果,分析ECG、UCG检查诊断HCM的灵敏度、特异度,建立接受者操作特征(ROC)曲线并计算ROC曲线下面积(AUC)。结果:165例HCM患者中室间隔肥厚84例、左心室局限性或弥漫性增厚52例、心尖部肥厚29例,ECG诊断HCM的灵敏度为72.13%、特异度为78.26%、AUC为0.805;UCG诊断HCM的灵敏度为78.38%、特异度为85.60%、AUC为0.847,UCG诊断效能大于ECG。结论:ECG、UCG对HCM的诊断准确率均较高,可为HCM早期筛查提供参考依据,但UCG诊断效能大于ECG。Objective:To explore the value of echocardiography(UCG)and electrocardiogram(ECG)in the diagnosis of hypertrophic cardiomyopathy(HCM).Methods:The clinical data of 165 patients with HCM in Fujian provincial hospital were retrospectively analyzed.All patients received ECG and UCG examination and gave the diagnosis results.The sensitivity and specificity of ECG and UCG examination in the diagnosis of HCM were analyzed.Receiver operating characteristic(ROC)curve was established and area under ROC curve(AUC)was calculated.Results:Among 165 patients with HCM,there were 84 cases of ventricular septal hypertrophy,52 cases of left ventricular local or diffuse thickening and 29 cases of apical hypertrophy.The sensitivity,specificity and AUC of ECG in the diagnosis of HCM were 72.13%,78.26%and 0.805 respectively;the sensitivity,specificity and AUC of UCG in the diagnosis of HCM were 78.38%,85.60%and 0.847 respectively.Conclusion:the diagnostic accuracy of ECG and UCG for HCM is high,which can provide reference for early screening of HCM,but the diagnostic efficiency of UCG is higher than that of ECG.
