摘  要:目的探讨Graves病(GD)患者甲状腺激素及甲状腺自身抗体与肾功能的相关性及临床意义。方法选取GD并甲亢患者329例设为甲亢组,健康体检者89例设为对照组,收集研究对象的甲状腺功能、肾功能及甲状腺自身抗体的检测结果,对两组的甲状腺功能和肾功能进行对比分析,对甲亢组的甲状腺功能、甲状腺自身抗体与肾功能相关指标进行相关性分析。结果甲亢组尿素、尿酸、肾小球滤过率(GFR)、游离三碘甲状腺原氨酸(FT 3)、游离甲状腺素(FT 4)、胱抑素C(Cys-C)、β_(2)-微球蛋白(β_(2)-MG)水平均高于对照组(P<0.05),肌酐、促甲状腺激素(TSH)水平低于对照组(P<0.05)。甲亢组FT 3、FT 4与肌酐均呈负相关(P<0.05),与尿酸、GFR、Cys-C、β_(2)-MG均呈正相关(P<0.05)。TSH与肌酐呈正相关(P<0.05),与尿酸、GFR、Cys-C、β_(2)-MG均呈负相关(P<0.05)。抗甲状腺过氧化物酶(TPOAB)与尿酸、GFR、β_(2)-MG均呈正相关(P<0.05)。抗甲状腺球蛋白抗体(TGAB)与尿素、肌酐呈负相关(P<0.05),促甲状腺素受体抗体(TRAB)与GFR、β_(2)-MG呈正相关(P<0.05)。TRAB与肌酐呈负相关(P<0.05),与尿酸、GFR呈正相关(P<0.05)。甲状腺刺激性抗体(TSAB)与肌酐呈负相关(P<0.05),与尿酸、GFR、Cys-C、β_(2)-MG均呈正相关(P<0.05)。结论GD患者的肾功能各项指标与健康对照者有差异,且其甲状腺激素及甲状腺自身抗体水平与肾功能各项指标的变化关系密切。Objective To analyze the relationship between thyroid hormone,thyroid autoantibodies and renal function and its clinical significance in patients with Graves’disease(GD).Methods A total of 329 cases of patients with GD and hyperthyroidism were selected for hyperthyroidism group,89 cases of healthy people as the control group.The test results of thyroid function,renal function and thyroid autoantibodies of the subjects were collected,and the thyroid function and renal function of the two groups were contrastively analyzed,and the correlation analysis of thyroid function,thyroid autoantibodies and renal function related indicators in the hyperthyroidism group were conducted.Results The levels of urea,uric acid,glomerular filtration rate(GFR),free triiodothyronine(FT 3),free thyroxine(FT 4)and cystatin C(Cys-C),β_(2)-microglobulin(β_(2)-MG)in hyperthyroidism group were higher than those in control group(P<0.05),and the levels of creatinine and thyroid stimulating hormone(TSH)in hyperthyroidism group were lower than those in control group(P<0.05).FT 3 and FT 4 in hyperthyroidism group were negatively correlated with creatinine(P<0.05),and positively correlated with uric acid,GFR,Cys-C andβ_(2)-MG(P<0.05).TSH was positively correlated with creatinine(P<0.05),and negatively correlated with uric acid,GFR,Cys-C andβ_(2)-MG(P<0.05).Thyroid peroxidase antibody(TPOAB)was positively correlated with uric acid,GFR andβ_(2)-MG(P<0.05).Thyroglobulin antibody(TGAB)was negatively correlated with urea and creatinine(P<0.05),and positively correlated with GFR andβ_(2)-MG(P<0.05).Thyrotropin receptor antibody(TRAB)was negatively correlated with creatinine(P<0.05),and positively correlated with uric acid and GFR(P<0.05).Thyroid stimulating antibody(TSAB)was negatively correlated with creatinine(P<0.05),and positively correlated with uric acid,GFR,Cys-C andβ_(2)-MG(P<0.05).Conclusion The renal function indexes of GD patients were different from those of healthy controls,and the levels of thyroid hormone and thyroid autoantibod
