慢性心力衰竭患者血清正五聚蛋白3、成纤维细胞生长因子2的表达水平及其与心肌重构和心功能的相关性

摘　　要：目的检测慢性心力衰竭(CHF)患者血清正五聚蛋白3(PTX3)、成纤维细胞生长因子2(FGF2)的表达水平,分析二者与心肌重构和心功能的关系。方法选取2018年4月至2019年5月秦皇岛军工医院(以下简称“我院”)89例CHF患者为研究对象,并将其作为实验组,我院同期90名健康体检者作为对照组。根据患者心功能状态,将CHF患者分成Ⅰ级(14例)、Ⅱ级(18例)、Ⅲ级(31例)、Ⅳ级(26例)。检测纳入对象血清中PTX3、FGF2水平;分析心功能分级与血清中PTX3、FGF2水平的关系;分析CHF患者血清中PTX3、FGF2水平与心功能、心肌重构指标关系。结果实验组血清PTX3、FGF2水平高于对照组(P<0.05)。CHF患者中,与Ⅰ级比较,Ⅱ、Ⅲ、Ⅳ级患者血清中PTX3、FGF2水平升高(P<0.05);与Ⅱ级比较,Ⅳ级患者血清中PTX3、FGF2水平升高(P<0.05);与Ⅲ级比较,Ⅳ级患者血清中PTX3水平升高(P<0.05)。与Ⅰ级比较,Ⅱ、Ⅲ、Ⅳ级患者左心室舒张末期内径(LVEDD)、左心房内径(LAD),Ⅳ级患者左心室质量指数(LVMI)升高,Ⅲ、Ⅳ级患者左心室心输出量(LVCO),Ⅳ级患者左心室射血分数(LVEF)、左室重构指数(LVRI)降低(P<0.05)。与Ⅱ级比较,Ⅳ级患者LVEDD、LAD升高,Ⅲ、Ⅳ级患者LVCO,Ⅳ级患者LVEF降低(P<0.05)。与Ⅲ级比较,Ⅳ级患者LVEDD、LAD升高,LVEF、LVCO降低(P<0.05)。CHF患者血清中PTX3水平与LVEDD、LVMI呈正相关,与LVEF、LVCO呈负相关(P<0.05);FGF2水平与LVEDD、LAD、LVMI呈正相关,与LVEF、LVRI呈负相关(P<0.05)。结论CHF患者血清中PTX3、FGF2水平升高,与心功能、心肌重构指标关系密切,影响疾病严重程度。Objective To detect the expression levels of serum pentagerin 3(PTX3)and fibroblast growth factor 2(FGF2)in patients with chronic heart failure(CHF),and to analyze the relationship between them and myocardial remodeling and cardiac function.Methods A total of 89 CHF patients in Qinhuangdao Jungong Hospital(hereinafter referred to as“our hospital”)from April 2018 to May 2019 were selected as the research subjects,and they were taken as the experimental group,and 90 healthy subjects in our hospital during the same period were taken as the control group.According to cardiac function status,CHF patients were divided into gradeⅠ(14 cases),gradeⅡ(18 cases),gradeⅢ(31 cases),and gradeⅣ(26 cases).Serum PTX3 and FGF2 levels of the included subjects were detected.The relationship between cardiac function grade and serum PTX3 and FGF2 levels was analyzed.The relationship between the serum levels of PTX3,FGF2 and cardiac function and myocardial remodeling in CHF patients was analyzed.Results Serum levels of the serum level of PTX3 and FGF2 in experimental group were higher than those in the control group(P<0.05).In patients with CHF,compared with gradeⅠ,serum levels of PTX3 and FGF2 were increased in patients with gradesⅡ,Ⅲ,Ⅳ(P<0.05).Compared with gradeⅡ,the serum levels of PTX3 and FGF2 were increased in patients with gradeⅣ(P<0.05).Compared with gradeⅢ,the serum levels of PTX3 and FGF2 were increased in patients with gradeⅣ(P<0.05).Compared with gradeⅠ,the left ventricular end diastolic dimension(LVEDD)and left atrium diameter(LAD)in patients with gradesⅡ,Ⅲ,Ⅳ,left ventricular mass index(LVMI)in patients with gradeⅣwere increased,left ventricular cardiac output(LVCO)in patients with gradesⅢ,Ⅳ,left ventricular ejection fraction(LVEF)and left ventricular reconstruction index(LVRI)in patients with gradeⅣwere decreased(P<0.05).Compared with gradeⅡ,the LVEDD and LAD in patients with gradeⅣwere increased,the LVCO in patients with gradesⅢ,Ⅳ,the LVEF in patients with gradeⅣwere d
