摘 要:目的探讨苏州地区中青年女性人群中腰椎(L_(1~4))骨密度与总胆固醇(TC)、甘油三脂(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)水平以及血红蛋白、白蛋白、体质量指数(body mass index,BMI)之间的关系。方法回顾性纳入接受健康体格检查的中青年女性4911名,按照年龄分为≤30岁,30~40岁组,40~50岁组,入选者进行组间各变量均值的方差分析以及各因素的Person相关分析、偏相关分析和多元线性逐步回归分析。结果中青年女性人群中,腰椎(L_(1~4))骨密度与TC呈负相关(r=-0.043,P=0.003),与HDL-C也存在负相关(r=-0.065,P<0.001),与BMI存在正相关(r=0.167,P<0.001),与年龄存在负相关(r=-0.041,P=0.004);排除年龄和BMI两因素后的偏相关分析提示,只有TC和腰椎骨密度呈负相关,并有统计学意义(r=-0.046,P=0.001);多元线性逐步回归分析提示,年龄、BMI、TC以及LDL-C能进入回归模型(R^(2)=0.037)。结论中青年女性的腰椎骨密度与年龄、BMI、TC有明显相关性,而且TC与腰椎骨密度存在显著负相关。因此,对于血脂总胆固醇偏高的中青年女性存在骨密度降低甚至骨质疏松的风险,在防治骨质疏松症的过程中要注意关注和控制血清总胆固醇水平。Objective To explore the relationship between bone mineral density(BMD)of lumbar spine(L_(1-4))and total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDLC),as well as hemoglobin,albumin and body mass index(BMI)in the young or middle-aged women in suzhou area.Methods A retrospective study was conducted on 4911 middle-aged and young females who had the healthy physical examination,they were divided into three groups according to age:≤30 years old group,30-40 years old group and 40-50 years old group.The selected subjects were selected to conduct anova of the mean values of all variables between the groups,as well as person correlation analysis,partial correlation analysis and multiple linear stepwise regression analysis of all factors.Results Lumbar spine(L_(1-4))BMD was negatively correlated with total cholesterol(r=-0.043,P=0.003)and negatively correlated with hdl-c(r=-0.065,P<0.001)in young and middle-aged women,with statistical significance.In addition,lumbar spine BM D was positively correlated with BMI(r=0.167,P<0.001)and negatively correlated with age(r=-0.041,P=0.004).Partial correlation analysis after the exclusion of age and BMI indicated that only TC was negatively correlated with lumbar spine BM D,which was statistically significant(r=-0.046,P=0.001).Multiple linear stepwise regression analysis suggested that age,BMI,TC,and LDL-C could enter the model(R^(2)=0.037).Conclusion BMD of lumbar spine is significantly correlated with age,BMI and TC in young and middle-aged women,and TC is significantly negatively correlated with lumbar spine BMD.Therefore,middle-aged and young women with high blood lipid and total cholesterol are at risk of bone mineral density reduction and even osteoporosis.So in the process of prevention and treatment of osteoporosis,should pay attention to the serum total cholesterol level.
