Toll样受体及细胞因子表达水平与肠道病毒感染患儿临床表型的相关性分析

摘　　要：目的探讨细胞因子水平及Toll样受体7基因单核昔酸多态性(SNP)与肠道病毒感染不同临床表型之间的相关性。方法对200例肠道病毒感染患儿和100例健康对照者TLR7基因rs3853839位点进行基因分型,比较其等位基因频率的差异,运用流式细胞术检测T淋巴细胞亚群及细胞因子水平。结果EV71感染者rs3853839基因型GC和CC的频率明显高于其它型别感染组(P<0.05);重症组rs3853839基因型C的频率明显高于轻症组(P<0.05);重症组患儿CD3^(+)、CD4^(+)、NK细胞数量明显低于轻症组;重症组与轻症组患儿B淋巴细胞数量无明显差异。肠道病毒感染重症组患儿血清中IL-10、IL-6、IFN-γ水平显著高于轻症组。结论TLR7基因rs3853839等位基因C与是儿童EV71感染的危险因素,且与临床症状重症化相关;淋巴细胞及细胞因子异常可能与肠道病毒感染患者重症化及预后相关。Objective To investigate the relationship between single nucleotide polymorphism(SNP)of Toll like receptor 7(TLR7)gene and different clinical phenotypes.Methods Genotyping was performed on 200 children with enterovirus infection and 100 healthy controls at the rs3853839 locus of TLR7 gene,and the differences in allele frequencies were compared.T lymphocyte subsets and cytokine levels were detected by flow cytometry.Results EV71 infection rs3853839 genotype GC and CC frequency were significandy higher than other types of infection group(P<0.05).The C frequency of rs3853839 genotype in Encephalitis/aseptic meningitis group was also significantly higher than other group(P<0.05).The number of CD3^(+),CD4^(+)and NK ceUs in severe group was significantly lower than that in mild group(P<0.05).On the contrary,the number of IL-10 and IL-6 in severe group was significantly higher than that in mild group(P<0.05).Conclusion TLR7 rs3853839 allele C is a risk factor for EV71 infection in children,and the frequency of allele C is associated with the severity of clinical symptoms.
