摘 要:目的精准把控术前预防性使用抗生素的时机,提高术前抗生素使用合格率。方法选取本院2019年6月至2019年8月术前有使用抗生素的手术2373台作为对照组,2019年10月至2019年12月术前有使用抗生素的手术3593台作为观察组。构建术前预防性抗生素管理信息融合系统,通过完善信息发布系统,增设抗生素用药管理模块、术前抗生素登记模块、术前抗生素统计模块以达到精确时间管理干预术前抗生素的用药时机。统计术前抗生素使用时间合格率、执行术前抗生素的平均用时及术前抗生素使用不合格的原因。结果两组术前抗生素使用时间合格率比较,差异有统计学意义(P<0.01)。观察组术前抗生素执行、登记、统计的平均用时均短于对照组。导致术前抗生素使用不合格的原因有麻醉医生气管插管困难、CVC穿刺困难、辅助工人或手术医生不到位、术前准备复杂、前一台手术延误、呼叫接台过晚等。结论应用信息技术优化手术信息管理系统并增加术前抗生素用药管理模块有助于提高术前预防性抗生素使用时间的合格率、提升手术室护士的工作效率以及术前抗生素使用的精准化管理。Objective To precisely control the timing of preventive use of antibiotics before surgery,and to improve the qualification rate of antibiotic use before surgery.Methods We selected 2,373 preoperative antibiotic surgeries from June 2019 to August 2019 in our hospital as the control group,and 3,591 preoperative antibiotic surgeries from October 2019 to December 2019 as the observation group.Construct a preoperative preventive antibiotic management information fusion system,and improve the information release system to add an antibiotic medication management module,a preoperative antibiotic registration module,and a preoperative antibiotic statistics module to achieve precise time management and intervention of preoperative antibiotic medication timing.Statistics of the preoperative antibiotic use time qualified rate,the average preoperative antibiotic use time and the reasons for the preoperative antibiotic use unqualified were counted.Results There was a statistically significant difference in the preoperative antibiotic use time qualification rate between the two groups(P<0.01).The average time for preoperative antibiotic execution,registration,and statistics in the observation group was shorter than that in the control group.The reasons leading to the unqualified use of preoperative antibiotics included difficulty in tracheal intubation by the anesthesiologist,difficulty in puncturing the CVC,inadequate auxiliary workers or surgeons,complicated preoperative preparations,delays in the previous operation,and late calls to take over.Conclusion The application of information technology to optimize the surgical information management system and increase the preoperative antibiotic medication management module will help improve the qualification rate of preoperative preventive antibiotic use time,improve the efficiency of the operating room nurses,and the precise management of preoperative antibiotic use.
