改良指端放血法在断指再植静脉危象中的应用

摘　　要：目的探讨改良指端放血法在断抬再植静脉危象中的临床应用效果。方法84例断指再植静脉危象患者根据入院时间分为传统组和改良组,毎组各42例。传统组采用指端侧切口放血法,改良组采用指腹按摩结合指端真皮浅层创面放血法。观察两组患者手指成活率、放血时间、放血创面出血自凝时间、创口愈合时间以及再植手指外形满意度。结果传统组手指成活率为76.19%,改良组手扌旨成活率为92.86%,差异有统计学意义(P<0.05)。改良组与传统组放血时间、放血创面出血自凝时间比较,差异无统计学意义(P>0.05)。改良组创口金合时间短于传统组(P<0.05)。改良组患者对再植手指外形满意度的评分高于传统组(P<0.05)。结论改良指端放血法可有效提高断指再植静脉危象患者的手指成活率,且在同样放血时间和凝血时间的条件下,能够取得更好的恢复效果和患者满意度。Objective To explore the clinical effect of improved fingertip bleeding method in venous crisis of replantation of severed finger.Methods The 84 patients with severe finger replantation venous crisis were divided into the traditional group and the modified group according to the time of admission,with 42 cases in each group.The traditional group was used finger tip side incision bloodletting method,and the modified group was used fingertip massage combined with finger tip dermal superficial wound bloodletting method.The finger survival rate,bloodletting time,bloodletting wound self-coagulation time,wound healing time,and satisfaction with the appearance of replanted fingers were observed.Results The survival rate of fingers in the traditional group was 76.19%,and 92.86% in the improved group,with significant difference(P<0.05).There was no significant difference in bleeding time and bleeding self-coagulation time between the improved group and the traditional group(P>0.05),The healing time of wounds in the improved group was lower than that in the control group(P<0.05),and the satisfaction score of replanted fingers in the improved group was higher than that in the traditional group(P<0.05).Conclusion The improved finger exsanguination method can efiectively improve the finger survival rate of patients with severed finger replantation venous crisis,and under the same conditions of bloodletting time and clotting time,it can achieve better recovery effect and patient satisfaction.
