彩色多普勒超声观察缺血性眼病患者颈动脉支架置入术后患眼的血流动力学改变

摘　　要：目的观察颈动脉支架置入术(CAS)治疗颈动脉狭窄性缺血性眼病(IOP)对患眼血流动力学的影响。方法38例符合CAS手术指征的IOP单眼患者,分别于手术前、术后3天及6个月行眼球彩色多普勒超声(CDU)检查,观察所有患眼的眼动脉(OA)、视网膜中央动脉(CRA)和睫状后短动脉(PCA)的收缩期峰值血流速度(PSV)、舒张末期血流速度(EDV)、阻力指数(RI)和博动指数(PI)等血流动力学指标变化。结果与CAS术前比,术后3天、6个月患眼OA、CRA和PCA的PSV、EDV值增加(P<0.05),术后3天、6个月患眼OA和PCA的RI、PI值减少(P<0.05),术后3天患眼CRA的RI、PI值减少(P<0.05)。CAS术后患眼频谱形态恢复正常,13例OA反向血流恢复正向血流。结论颈动脉狭窄性IOP患者行CAS后,眼部血供明显改善,CDU可应用于治疗效果的评估。Objective To observe the effect of carotid artery stenting(CAS)on the hemodynamics of carotid artery stenosis ischemic ophthalmopathy(IOP).Methods Thirty-eight patients with IOP monocular who were in line with the indications for CAS surgery underwent eye color Doppler ultrasound(CDU)examination before surgery,3 days after surgery,and 6 months after surgery to observe the ophthalmic artery(OA)and central retina of all the affected eyes.Hemodynamic indicators such as peak systolic blood flow velocity(PSV),end-diastolic blood flow velocity(EDV),resistance index(RI)and pulsatility index(PI)of arteries(CRA)and short posterior ciliary artery(PCA)Variety.Results Compared with before operation,the PSV and EDV values of OA,CRA and PCA of the affected eye had increased at 3 days and 6 months after operation(P<0.05),and the RI and PI values of OA and PCA of the afiected eye had decreased at 3 days and 6 months after operation(P<0.05),the RI and PI values of CRA of the affected eye at 3 days after the operation had decreased(P<0.05).The spectrum morphology o£the affected eye returned to normal after the operation,and the reverse blood flow of 13 cases of OA returned to the positive blood flow.Conclusion The blood supply to the eyes of patients with carotid artery stenosis IOP is significantly improved after CAS,and CDU can be used to evaluate the therapeutic efiect.
