云南高原汉族农村老年人重大生活事件与生命质量的关联性分析

摘　　要：目的了解云南高原汉族农村老年人生命质量现状和近两年的重大生活事件发生状况,探讨两者之间的相关性。方法基于中国多民族队列中的高原汉族队列的基线调查数据,采用英国的EQ-5D-5L效用值积分体系计算健康效用值作为评价生命质量的指标,采用中位数回归分析10种重大生活事件与老年人生命质量的相关性。结果基于英国效用值积分体系所得的健康效用值中位数得分为0.942分。重大生活事件发生率为28.5%,重大生活事件是农村老年人健康相关生命质量的影响因素(P<0.001)。对性别、年龄、婚姻状况等因素进行调整后,中位数回归分析结果显示,严重的家庭内部矛盾及冲突(β=-0.056,P=0.007)和家庭其他成员死亡或患重病(β=-0.028,P=0.034)是农村老年人生命质量的影响因素。结论经历家庭内部矛盾与冲突、家庭其他成员死亡或患重病等重大生活事件会降低农村老年人的生命质量。Objective To know about the status quo of quality of life among the rural Han elderly in Yunnan Plateau and the occurrence of major life events in recent two years,and to explore the correlation between them.Methods Based on the baseline survey data of the plateau Han nationality cohort in the China Multi-Ethnic cohort,the EQ-5 D-5 L utility value scoring system of the United Kingdom was used to calculate the health utility value as an index to evaluate the quality of life,and the correlation between 10 major life events and the quality of life of the elderly was analyzed by median regression model.Results The median of health utility value calculated based on British utility value integral system was 0.942.The incidence of major life events was 28.5%,and major life events were the factors influencing the health-related quality of life of the elderly in rural areas(P<0.001).After adjusting the factors including gender,age and marital status,median regression analysis results showed that severe intra-family conflicts(β=-0.056,P=0.007)and death of other family members or other family members suffering from serious illness(β=-0.028,P=0.034)were influence factors for the quality of life of the rural elderly.Conclusions Encountering major life events,especially experiencing contradictions and conflicts within the family,other family members’death or their being seriously ill will reduce the quality of life of the rural elderly.
