摘  要:目的了解利用不同方法检测痰标本的结果对比情况。方法选取615例肺结核病患者痰液,进行涂片、培养、GeneXpert MTB/RIF检测,对不同检测方法所得结果对比分析。应用比例法、基因芯片法、GeneXpertMTB/RIF检测结核杆菌的耐药情况。结果615例肺结核患者经痰涂片、痰培养、GeneXpert MTB/RIF检测,阳性率分别为28.62%、44.07%和57.07%,两两比较差异均有统计学意义(P<0.05)。GeneXpert MTB/RIF检测结核杆菌“高”和“中”量级共221例,涂片检测46例阴性,“低”和“极低”量级共130例,涂片镜检均为阴性。比例法、基因芯片法、GeneXpertMTB/RIF检测利福平耐药率分别为13.28%、21.77%、22.14%;比例法、基因芯片法检测异烟肼耐药率分别为16.97%、25.46%;2种药物的药敏结果差异均有统计学意义(P<0.05)。结论GeneXpert MTB/RIF能提高阳性率。GeneXpert MTB/RIF和基因芯片法能够快速筛查耐药结核杆菌,对耐多药结核的诊断意义重大。Objective To investigate the detection results of sputum samples using different methods.Methods The sputum of 615 patients with tuberculosis was selected for smear,culture,and GeneXpert MTB/RIF detection.The results of different detection methods were compared and analyzed.Proportional method,genechip,and GeneXpertMTB/RIF were used to detect the resistance of tuberculosis.Results The positive rates of tuberculosis in 615 patients were 28.62%,44.07%and 57.07%by sputum smear,sputum culture and GeneXpert MTB/RIF,respectively,with the difference statistically significant in pair comparison(P<0.05).221 cases were detected by GeneXpert MTB/RIF at“high”or“medium”magnitude,46 cases were negative by smear detection,130 cases were detected at“low”or“very low”magnitude,and all of them were negative by smear microscopy.The drug resistance rates of rifampicin detected by proportional method,gene chip method and GenExpert-MTB/RIF were 13.28%,21.77%and 22.14%,respectively.The drug resistance rates of isoniazid detected by proportional method and gene chip method were 16.97%and 25.46%,respectively.There was statistical significance on the differences in drug sensitivity between the two drugs(P<0.05).Conclusion GeneXpert MTB/RIF can improve the positive rate.GeneXpert MTB/RIF and gene chip method can quickly screen drug-resistant tuberculosis bacteria,which is of great significance for the diagnosis of MDR-TB.
