高尿酸血症和痛风患者HLA-B*5801携带率及其合并症相关性研究

摘　　要：目的了解高尿酸血症和痛风患者HLA-B*5801基因的阳性率,指导患者临床用药及基因筛查的必要性。方法选取212例需服用别嘌呤醇的高尿酸患者,其中高尿酸血症者157例,痛风患者55例。采用序列特异性的寡核苷酸探针LABType SSO(sequence-specific oligonudeotide,SSO)技术对患者血样DNA进行HLA-B*5801基因检测。结果入组的患者中HLA-B*5801基因阳性例数为49例,阳性率为23.11%,HLA-B*5801基因阳性率在性别、年龄和疾病之间比较差异无统计学意义(P>0.05);49例阳性者,21例在基因检测前服用了别嘌呤醇,其中10例发生皮肤过敏反应,另11例中有1例发生不可逆皮肤过敏反应SJS/TEN。2组患者血尿酸水平差异无统计学意义(P>0.05),与高尿酸血症患者相比,痛风患者更容易并发高脂血症(P<0.05)。结论本地区高尿酸血症和痛风患者HLA-B*5801基因携带率高,患者在服用别嘌呤醇前进行HLA-B*5801基因检测十分必要,防止发生因服用别嘌呤醇导致的严重皮肤不良反应。Objective To understand the positive rate of HLA-B*5801 gene in patients with hyperuricemia and gout,and guide patients on the necessity of clinical medication and genetic screening.Methods A total of 212 hyperuric acid patients requiring allopurinol were selected,including 157 hyperuricemia patients and 55 gout patients.Sequence-specific oligonucleotide probe LABType SSO(Sequence-Specific Oligonudeotide,SSO)technology was used for HLA-B*5801 gene detection of DNA from patients.Results Among the enrolled patients,the number of HLA-B*5801 gene positive cases was 49,with a positive rate of 23.11%.There was no statistical significance on the difference in the positive rate of HLA-B*5801 gene between gender,age,and disease(P>0.05).Of the 49 positive cases,21 had taken allopurinol before the genetic test,of which 10 cases had skin allergic reactions,and 1 of 11 cases had irreversible skin allergic reactions SJS/TEN.There was statistical significance on the blood uric acid levels of the two groups(P>0.05).Compared with patients with hyperuricemia,patients with gout are more likely to have hyperlipidemia(P<0.05).Conclusion Patients with hyperuricemia and gout in this area have a high carrying rate of HLA-B*5801 gene.It is necessary for patients to perform HLA-B*5801 gene testing before taking allopurinol to prevent serious skin defects caused by allopurinol reaction.
