SysmexXN2000与XE5000自动计数外周血有核红细胞的准确性验证

摘　　要：目的验证XN-2000、XE-5000自动计数有核红细胞(NRBC)的准确性及临床应用价值。方法分别用XN-2000和XE-5000血细胞分析仪对200例EDTA-K2抗凝血标本做血常规检测,并制作血涂片,用显微镜计数每个标本的NRBC,并按仪器计数NRBC百分数结果将标本分为阴性组、低值组、中值组和高值组,每组各50例。结果XE-5000非有核红通道计数WBC与有核红通道、XN-2000结果比较,差异有统计学意义(P=0.00),且WBC校正前、后结果比较,差异有统计学意义(P<0.01),检测中值、低值NRBC时阳性报警率仅为25%;XE-5000有核红通道与XN-2000计数WBC结果比较,差异无统计学意义(P>0.05);XN-2000计数NRBC百分数与显微镜计数结果比较,差异无统计学意义(P>0.05),但呈显著正相关性(r=0.9952);XE-5000与显微镜计数结果比较,差异有统计学意义(P<0.05),呈显著正相关性(r=0.9921);两仪器计数NRBC均有较好重复性和良好线性关系。结论用XE-5000有核红通道或XN-2000检测可使WBC结果更准确,并获得NRBC值,且XN-2000计数NRBC的准确性优于XE-5000。不论标本中是否有NRBC,用XE-5000检测有核红通道很有必要。Objective To verify the accuracy and clinical application value of XN-2000 and XE-5000 automatic counting of nucleated red blood cells(NRBC).Methods Peripheral blood samples were analyzed by XN2000,XE5000 and microscope,respectively.According to the percent of NRBC,the results were divided into negative group,low value group,mid-value group and high value group,with 50 cases in each group.NRBC of these samples were also counted by microscope.Results The difference was statistically significant between the results of XE-5000 without NRBC count and XN-2000 with NRBC count(P=0.00);and the difference was statistically significant between the results before and after WBC correction(P<0.01).The positive alarm rate was only 25%when the median and low NRBC values were detected.There was no statistical significance on the difference in WBC results between XE-5000 and XN-2000(P>0.05).There was no statistical significance on the difference in NRBC percentage between XN-2000 count and microscope count(P>0.05),and there was a significant positive correlation(r=0.9952).The difference between XE-5000 and microscope counting results was statistically significant(P<0.05),but there was a significant positive correlation(r=0.9921).The NRBC counts of the two instruments have good repeatability and good linear relationship.Conclusion XE-5000 with nuclear red channel or XN-2000 detection can make WBC results more accurate and obtain NRBC value,and the accuracy of XN-2000 counting NRBC is better than that of XE-5000.Whether or not the NRBC is present in the specimen,it is necessary to use the nucleated red channel in the XE-5000 assay.
