摘 要:目的:探讨血液透析联合血液灌流对慢性肾功能衰竭(CRF)患者血清炎症递质、β2-微球蛋白(β2-MG)、甲状旁腺激素(PTH)水平的影响。方法:抽取泰安市第一人民医院2018年11月至2019年12月收治的90例CRF患者作为研究对象,采用交替分组法将其分为A组和B组,每组45例。A组采用血液透析治疗,B组在A组基础上联合血液灌流治疗。比较两组治疗前及治疗3个月后炎症递质[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)]、肾功能[血肌酐(Scr)、血尿素氮(BUN)]、血液净化指标(β2-MG、PTH)变化。结果:治疗后,两组IL-6、TNF-α、CRP水平均低于治疗前,且B组低于A组,差异有统计学意义(P<0.05)。治疗后,两组Scr、BUN水平均低于治疗前,且B组低于A组,差异有统计学意义(P<0.05)。治疗后,两组β2-MG、PTH水平均低于治疗前,且B组低于A组,差异有统计学意义(P<0.05)。结论:血液透析联合血液灌流可有效缓解CRF患者炎症反应,降低其β2-MG、PTH水平,改善肾功能。Objective To investigate the effects of hemodialysis combined with hemoperfusion on serum levels of inflammatory mediators,β2-microglobulin(β2-MG)and parathyroid hormone(PTH)in patients with chronic renal failure(CRF).Methods A total of 90 patients with CRF treated in the First People’s Hospital of Taian from November 2018 to December 2019 were selected as the research objects.And they were divided into group A and group B by alternating grouping method,with 45 cases in each group.Group A was treated by hemodialysis,and group B was treated by hemodialysis combined with hemoperfusion.The inflammatory mediators,such as interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),renal function assessed by serum creatinine(Scr),blood urea nitrogen(BUN),blood purification indexes(β2-MG,PTH)were compared between the two groups before and after treatment.Results After treatment,the levels of IL-6,TNF-αand CRP in the two groups were lower than those before treatment,and those in group B were lower than those in group A(P<0.05).After treatment,the levels of Scr and BUN in both groups were lower than those before treatment,and those in group B were lower than those in group A(P<0.05).After treatment,the levels ofβ2-MG and PTH in the two groups were lower than those before treatment,and those in group B were lower than those in group A(P<0.05).Conclusions Hemodialysis combined with hemoperfusion can effectively alleviate the inflammatory reaction,reduce the levels ofβ2-MG and PTH,and improve renal function.
