桃红四物汤对全膝关节置换术后疼痛和功能康复的影响

摘　　要：目的:探讨桃红四物汤对全膝关节术后疼痛和功能康复的影响。方法:抽取2019年6月至2020年6月于郑州市骨科医院行全膝关节置换术的患者83例,按随机数字表法分为观察组42例和对照组41例。两组患者均行单侧全膝关节置换术,手术由同一组医师完成。对照组采用全膝关节置换术后常规治疗方案;观察组在常规治疗方案的基础上口服桃红四物汤,每日1剂,早晚各1次,共服用2周。记录两组患者术前及术后14 d的视觉模拟评分法(VAS)评分、美国特种外科医院膝关节评分(HSS评分)、关节活动度(ROM)及丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)水平。结果:两组患者均获得随访。术前,两组VAS评分、HSS评分、关节活动度、ALT及AST水平比较差异未见统计学意义(P>0.05)。术后,观察组VAS评分为(5.21±1.26)分,低于对照组的(5.78±0.57)分(P<0.05);观察组HSS评分及ROM分别为(57.21±4.74)分、109.76°±7.07°,高于对照组的(53.78±5.34)分、106.49°±5.42°,P<0.05。术后,两组ALT、AST水平比较差异未见统计学意义(P>0.05)。结论:全膝关节置换术后应用桃红四物汤可明显减轻患者术后疼痛,改善关节活动度,利于患者术后快速康复。Objective To investigate the effect of Taohong Siwu decoction on postoperative pain and functional rehabilitation in patients after total knee arthroplasty.Methods A total of 83 patients who underwent total knee arthroplasty in Zhengzhou Orthopedics Hospital from June 2019 to June 2020 were selected and divided into observation group(42 cases)and control group(41 cases)by random number table method.Both groups underwent unilateral total knee arthroplasty,and the operations were performed by the same group of physicians.The control group was treated by conventional treatment;while the observation group was treated with Taohong Siwu decoction based on the treatment of conventional treatment,one dose daily,once in the morning and once in the evening,for 2 weeks.The visual analogue scale(VAS)score,hospital for special surgery knee score(HSS score),range of motion(ROM),the levels of alanine aminotransferase(ALT)and aspartate aminotransferase(AST)were recorded before and 14 days after operation.Results Both groups were followed up.Before operation,there was no significant difference in VAS score,HSS score,ROM,ALT and AST levels between the two groups(P>0.05).After operation,the VAS score of observation group(5.21±1.26)was significantly lower than that of control group(5.78±0.57),P<0.05;and HSS score and ROM of the observation group(57.21±4.74,109.76°±7.07°)were lower than those of the control group(53.78±5.34,106.49°±5.42°),P<0.05.After operation,there was no significant difference in ALT and AST levels between the two groups(P>0.05).Conclusions Application of Taohong Siwu decoction after total knee arthroplasty can significantly reduce the postoperative pain,improve the range of motion,and is conducive to the rapid recovery of patients.
