摘  要:目的探讨二维斑点追踪技术(2D-STI)在正常妊娠胎儿心脏功能应用价值。方法选取2018年8月至2019年10月在我院进行常规产检的正常孕妇86例为研究对象。获得胎儿24周、32周和37周的心尖四腔心二维图像,并测量胎儿左室及右室的整体纵向应变(GLS)和收缩期应变率(GLSr)、舒张早期应变率(GLEr)及舒张晚期应变率(GLAr),再对所得数据进行统计学分析。结果胎儿24、32及37周LVEDd、RVEDd、IVSd、LVPWd及E/A参数随孕周增大而增大,各孕周比较差异具有统计学意义(P<0.05)。各孕周与24周LVGLEr、RVGLS及RVGLSr参数比较(P<0.05),与各孕周LVGLEr参数比较(P<0.05)。结论2D-STI能很好的反应心脏应变及应变率变化及生理变化,并获得左心室心肌的运动参数。Objective To explore the application value of two-dimensional speckle tracking technology(2 D-STI)in normal pregnancy fetal heart function.Methods A total of 86 normal pregnant women who underwent routine check-ups in our hospital from August 2018 to October 2019 were selected.The two-dimensional images of the apical four-chamber heart in the fetus were obtained at 24,32,and 37 weeks.Global longitudinal strain(GLS),systolic strain rate(GLSr),and early diastolic strain rate(GLEr)and late diastolic strain rate(GLAr)in the left and right ventricles of the fetus were measured,and then statistical analysis was performed accordingly.Results LVEDd,RVEDd,IVSd,LVPWd and E/A parameters increased with the increase of gestational age at 24,32 and 37 weeks,and the difference in each gestational week was statistically significant(P<0.05).The difference inLVGLEr,RVRLS and RVRLSr parameters between each gestational week and 24 weeks was significant(P<0.05),and so was theLVGLEr parameters(P<0.05).Conclusion 2 D-STI can well reflect the heart strain,strain rate and physiologic changes,and obtain the motion parameters of left ventricular myocardium.
