程丑夫从少阳论治胸痹经验

摘　　要：总结程丑夫教授从少阳论治胸痹的经验。认为胸痹作为一种阵发性疾病,与少阳病的发病特点相似,且少阳经络、少阳脏腑均与心有密切关系。胸痹之病理因素痰、瘀的形成也与少阳枢机不利有关,因此可从少阳论治胸痹。临证以小柴胡汤为基础方和解少阳、疏肝理气以行气滞,兼有痰热,常选用柴胡陷胸汤清热化痰、宽胸散结以消痰热;兼有瘀血,选用柴胡丹参饮活血化瘀、行气止痛以散瘀血;若同时兼有痰热、瘀血,多选用柴陷丹参饮或柴胡加龙骨牡蛎汤加减。This paper summarized professor CHENG Choufu′s experience in treating chest bi(痹)based on shaoyang(少阳)theory.Chest bi is considered to be paroxysmal and has similar pathological characteristics with shaoyang diseases.Conversely,shaoyang channels,collaterals and zang-fu organs(脏腑)are all closely related to the heart.The abnormal performance of shaoyang pivot leads to the formulation of phlegm and stasis,which are the pathological factors of chest bi.Therefore,shaoyang theory can followed when treating chest bi.Xiaochaihu Decoction(小柴胡汤)is recommended as the basic formula in clinical practice,with the function of harmonizing shaoyang,soothing the liver and rectifying qi to move qi stagnation;when phlegm heat is accompanied,Chaihu Xianxiong Decoction(柴胡陷胸汤)is commonly used to disperse the phlegm heat through clearing heat and dissolving phlegm,loosening the chest and dissipating masses;when blood stasis is accompanied,Chaihu Danshen Decoction(柴胡丹参饮)is used to invigorate blood and dissolve stasis,move qi and relieve pain;when there are both phlegm heat and blood stasis,modified Chaixian Danshen Decoction(柴陷丹参饮)or Chaihu Longgu Muli Decoction(柴胡加龙骨牡蛎汤)are recommended.
