摘 要:目的观察传导型敷料Drawtex在深Ⅱ度烧伤创面治疗中的临床疗效。方法纳入2018年1月至2020年1月北京积水潭医院烧伤科收治的烧伤面积为5%~20%体表总面积(TBSA)的深Ⅱ度烧伤患者200例,随机分为观察组(n=100)和对照组(n=100)。两组均给予创面清创换药处理和凡士林油纱包扎常规治疗,观察组在常规治疗基础上应用Drawtex敷料。记录两组患者的创面愈合时间、创面愈合率、敷料增重、疼痛情况、创面细菌培养情况及不良反应。结果观察组创面愈合时间较对照组显著缩短[(18.47±3.21)d比(21.15±4.68)d,t=24.309,P=0.000]。伤后7、14、21及28 d观察组创面愈合率均显著高于对照组(均P<0.05)。第1次及第2次清创换药,观察组敷料增重均显著高于对照组,且观察组48 h内敷料平均增重显著高于对照组(均P<0.05)。伤后2、3、4 d换药后疼痛评估显示,观察组疼痛评分较对照组均显著降低(均P<0.05)。伤后3、7 d观察组创面细菌检出率均明显低于对照组(均P<0.05)。两组患者均未出现明显不良反应。结论应用传导型敷料Drawtex可改善深Ⅱ度烧伤创面的愈合情况。Objective To investigate the clinical outcomes of a hydroconductive dressing,Drawtex in treatment of deep second-degree burn wounds.Methods Include in this study were 200 patients with deep second-degree burns wounds on 5%to 20%of the total body surface area(TBSA)admitted to the Department of Burns,Beijing Jishuitan Hospital between January 2018 and January 2020.The patients were randomized to the study group(n=100)and the control group(n=100).Both groups were routinely handled with wound debridement,dressing change and coverage with vaseline oil gauze.In the study group,Drawtex was used in addition to the routine treatment.The time to complete healing,rate of wound healing,gains in dressing weight,pain,bacterial culture of wounds and adverse reactions were recorded for the two groups.Results The time to complete healing was significantly shorter in the study group than that in the control group[(18.47±3.21)d vs(21.15±4.68)d,t=24.309,P=0.000].At 7,14,21 and 28 days after injury,the rates of wound healing of were significantly higher in the study group than those in the control group(all P<0.05).In the first and second sessions of debridement and dressing change,the gain in dressing weight was significantly higher in the study group than that in the control group;so was the mean gain in dressing weight within 48 hours in the study group compared with the control group(all P<0.05).The scoring of pain assessed after dressing change on the days 2,3,and 4 days from injury was significantly lower in the study group than those in control group(all P<0.05).The detection yields of bacteria on wounds were significantly lower in the study group compared with the control group at 3 and 7 days from injury(both P<0.05).There were no evident adverse reactions in the two groups of patients.Conclusion Use of the hydroconductive dressing Drawtex can improve healing of deep second-degree burn wounds.
