摘　　要：目的探讨老年女性高血压患者左心室肥厚(LVH)与肱踝脉搏波传导速度(baPWV)的关系。方法入选于天津市人民医院心内科就诊的老年女性原发性高血压患者162例为高血压组,选择同期就诊的老年女性正常血压者40例为对照组,进行心脏彩色超声检查,计算左心室质量指数(LVMI)、相对室壁厚度(RWT),通过动脉硬化检测获得baPWV等指标。采用Spearman相关性分析,进行多元线性回归分析,并使用ROC曲线分析baPWV对于LVH的诊断价值。结果高血压组LVMI和baPWV高于对照组(P<0.01)。高血压组患者baPWV与年龄、收缩压、舒张压、脉压和LVMI呈显著正相关(P<0.01);多元线性回归分析显示,年龄、收缩压、舒张压和LVMI均是baPWV的影响因素(P<0.01)。ROC曲线分析显示,baPWV诊断LVH的曲线下面积为0.752,临界值为1601 cm/s (95%CI:0.679~0.825,P<0.01),进一步logistic回归分析显示,baPWV与高血压患者发生LVH相关(OR=1.501,95%CI:1.242~1.812,P<0.01)。结论老年女性高血压患者LVH与baPWV显著相关,临床上可用baPWV评估此类患者LVH的发生情况。Objective To study the relationship between LVH and baPWV in elderly female hypertension patients.Methods One hundred and sixty-two female essential hypertension patients served as a hypertension group and 40 elderly females with a normal blood pressure served as a control group.Their LVMI and RWT were calculated through echocardiography.Their baPWV was tested and their arterial stiffness was assessed by Spearman correlation analysis and multivariate linear regression analysis respectively.The value of baPWV for diagnosing LVH was analyzed by ROC curve analysis.Results The LVMI was significantly higher and the baPWV was significantly faster in hypertension group than in control group(P<0.05).The baPWV was positively related with age, SBP,DBP,PP and LVMI in hypertension group(P<0.01).Multivariate linear regression analysis showed that age, SBP,DBP and LVMI were the risk factors for baPWV in elderly female hypertension patients(P<0.01).ROC curve analysis displayed that the AUC for baPWV in diagnosing LVH in elderly female hypertension patients was 0.752 with critical value of 1601 cm/s(95%CI:0.679-0.825,P<0.01).Logistic regression analysis revealed that baPWV was related with LVH in elderly female hypertension patients(OR=1.501,95%CI:1.242-1.812,P<0.01).Conclusion LVH is closely related with baPWV and baPWV can predict LVH in elderly female hypertension patients.
