回顾性分析74例大咯血患者行支气管动脉栓塞术的安全性和有效性

摘　　要：目的探讨支气管动脉栓塞术在治疗大咯血中的应用。方法采用回顾性资料分析,选择我院2017年1月至2018年12月经支气管动脉栓塞治疗的大咯血患者74例,对该技术的安全性及疗效进行分析。结果74例患者均成功实施支气管动脉栓塞术,共栓塞126支咯血责任动脉,每例1~4支,其中1支31例,2支35例,3支7例,4支1例。在126支病变血管中,109支(86.5%)为支气管动脉,17支(13.5%)为非支气管体循环动脉。74例患者治愈63例,显效患者7例,有效2例,无效2例,止血总有效率为97.3%;复发3例,复发率为4.05%。术后5例出现发热,7例出现胸痛,经对症治疗后恢复正常,均未发生脊髓损伤及异位栓塞等严重并发症。结论支气管动脉栓塞术治疗大咯血疗效确切,复发率低,并发症少,安全性高。Objective To investigate the application of bronchial artery embolization in the treatment of massive hemoptysis.Methods By retrospective data analysis,74 patients with massive hemoptysis who were treated with bronchial artery embolization in our hospital from January 2017 to December 2018 were enrolled,and the safety and efficacy of this technique were analyzed.Results 74 patients were successfully treated with bronchial artery embolization,a total of 126 responsible arteries of hemoptysis were embolized,with 1 artery in 31 cases,2 arteries in 35 cases,3 arteries in 7 cases,and 4 arteries in 1 case.Of the 126 disaffected vessels,109(86.5%)were bronchial arteries and 17(13.5%)were non-bronchial arteries.In 74 patients,63 cases were cured,7 cases showed obvious effect,2 cases were effective and 2 cases were ineffective.The total effective rate of hemostasis was 97.3%.The recurrence rate was 4.05%.After the operation,5 patients developed fever and 7 patients developed chest pain.After symptomatic treatment,they recovered to normal.No serious complications such as spinal cord injury and ectopia embolism occurred in all patients.Conclusion Bronchial artery embolization is effective in the treatment of massive hemoptysis,with low recurrence rate,few complications and high safety.
