临床路径对老年性白内障手术平均住院日和平均住院费用的影响

摘　　要：目的分析临床路径管理对老年性白内障手术患者平均住院日和平均住院费用构成的影响,为临床路径的推广提供依据。方法选取某院2016年5月5日-2020年5月5日期间,白内障手术患者进入临床路径管理2301例,符合标准未纳入路径851例。比较2组患者平均住院日、费用等变化情况。结果实验组患者在平均住院日、床位费、诊查费、检查费、药费、卫生材料费、其他费用和总费用分别为4.01元、100.25元、20.36元、432.74元、204.28元、204.28元、1318.18元、284.25元、4127.12元,对照组分别为4.78元、119.5元、21.45元、458.32元、312.75元、1518.71元、321.22元、4531.3元,2组患者的差异有统计学意义(P<0.05);实验组患者治疗费、手术麻醉费、护理费分别为50.18、1673.24、43.64,对照组分别为51.15、1682.43、45.8,2组患者的差异没有统计学意义(P>0.05)。结论临床路径管理能缩短平均住院日,优化住院费用结构,降低住院费用。建议加强培训,转变理念;加强科室间协作,实行全流程管理;重视护理人员在临床路径实施中的作用。Objectives To analyze the effect of clinical pathway management on the average length of stay in hospital and the average cost of hospitalization in elderly cataract patients undergoing surgery,and to provide the basis for the promotion of clinical pathway.Methods From May 5th,2016 to May 5th,2020,a total of 2301 cataract surgery patients were enrolled in clinical pathway management,and 851 patients met the criteria and were not included in the pathway.The average length of stay and cost of patients in the two groups were compared.Results The average length of stay,bed cost,examination cost,medicine cost,sanitary material cost,other cost and total cost of patients in the experimental group were 4.01 yuan,100.25 yuan,20.36 yuan,432.74 yuan,204.28 yuan,204.28 yuan,1318.18 yuan,284.25 yuan,4127.12 yuan,respectively.The control group was 4.78 yuan,119.5 yuan,21.45 yuan,458.32 yuan,312.75 yuan,1518.71 yuan,321.22 yuan,4531.3 yuan,respectively,and the difference between the two groups was statistically significant(P<0.05).The treatment cost,surgical anesthesia cost and nursing cost in the experimental group were 50.18,1673.24 and 43.64,respectively,while those in the control group were 51.15,1682.43 and 45.8,respectively.There was no statistical significance between the two groups(P>0.05).Conclusions Clinical path management could shorten the average length of stay,optimize the structure of hospitalization cost and reduce the cost of hospitalization.It was suggested to strengthen training,change the concept;Strengthen the cooperation between departments,implement the whole process management;and pay attention to the role of nursing staff in the implementation of clinical pathways.
