摘　　要：目的探讨肺癌患者住院费用和影响因素,目的为减轻个人和社会经济负担和制定肺癌防治措施提供参考依据。方法选取哈尔滨市18家三级医院2019年1月1日-12月31日出院997例肺癌住院患者基本信息,并提取患者基本情况、住院情况和费用信息等,采用单因素方差分析和多元逐步回归分析法对住院费用的影响因素进行统计分析。结果单因素方差分析结果显示肺癌患者的住院费用影响因素包括支付方式、年龄、住院天数、医院类型、是否手术及药占比,均P<0.05,通过多元线性逐步回归方程得出肺癌住院费用的首要影响因素为是否手术,其次为住院天数;支付方式和医院类型在不同程度上均对住院费用产生影响。从肺癌患者住院费用构成来看,药品费、治疗费和耗材费占比分别为29.51%、28.99%和41.49%。结论适当的治疗措施和减少患者住院天数是降低肺癌患者住院费用的有效手段,通过建立规范的临床路径和加强耗材的管理,同时积极开展肺癌的三级预防等综合措施,可以有效地降低肺癌患者的住院费用。Objective To explore the hospitalization expenses and influencing factors of lung cancer patients,and to provide reference for reducing personal and social economic burden and formulating prevention and treatment measures of lung cancer.Methods The basic information of 997 hospitalized patients with lung cancer who were discharged from 18 tertiary hospitals in Harbin from January 1,to December 31,2019 was selected,and the basic information,hospitalization situation and cost information of the patients were extracted.The influencing factors of hospitalization expenses were analyzed by single factor analysis of variance and multiple stepwise regression analysis.Results The results of one-way ANOVA showed that the influencing factors of hospitalization expenses of lung cancer patients included payment method,age,length of stay,hospital type,operation and drug proportion,all P<0.05.Through multiple linear stepwise regression equation,it was concluded that the primary influencing factor of hospitalization expenses of lung cancer patients was operation,followed by length of stay;payment method and hospital type were opposite to each other in varying degrees Hospital costs have an impact.From the composition of hospitalization expenses of lung cancer patients,the proportion of drug expenses,treatment expenses and consumables expenses were 29.51%,28.99%and 41.49%respectively.Conclusion Appropriate treatment measures and reducing the length of stay are effective means to reduce the hospitalization expenses of lung cancer patients.Through the establishment of standardized clinical pathway,strengthening the management of consumables,and actively carrying out the three-level prevention of lung cancer,the hospitalization expenses of lung cancer patients can be effectively reduced.
