摘　　要：目的探讨尿液S100A6蛋白与细菌/真菌性尿路感染(urinary tract infection,UTI)的相关性。方法临床收集正常组和感染组尿液样本各110份,其中感染组患者具有典型的UTI症状,尿常规检测及尿液培养(细菌/真菌培养)结果均为阳性;正常对照组样本的尿液生化检查及尿液培养结果均为阴性。ELISA法检测尿液样本的A450值,Curve Expert软件计算S100A6蛋白的含量,应用SPSS 16.0软件进行Logistic回归分析,绘制ROC曲线,评价S100A6蛋白在尿路感染中的诊断价值。将感染组尿液样本于室温放置2、4、8、10、12、24、48、96 h,Western blot法检测尿液样本中S100A6蛋白含量的稳定性。结果感染组尿液样本中S100A6蛋白含量明显高于正常组(P <0.01)。Logistic回归方程为y=-0.971+0.04 x,OR值为1.004,P=0.001;ROC曲线下面积=0.862,约登指数=0.628,S100A6取临界值=23.277 pg/mL时,灵敏度为91.2%,特异度为71.6%。感染组尿液样本于室温放置2、4、8、10、12、24、48、96 h后,S100A6蛋白含量变化差异无统计学意义(P> 0.05)。结论 S100A6蛋白与UTI的相关性较大,具有潜在的UTI诊断价值,有望成为UTI初筛的新指标。Objective To investigate the correlation of S100 A6 protein in urine to bacterial or fungal urinary tract infection(UTI).Methods Urine samples were collected from 110 healthy persons and 110 patients with UTI.The patients showed typical symptoms of UTI,of whom the results of routine urinary test and urine culture(bacterial or fungi)were positive.However,both the results of urinary biochemical test and urine culture of healthy persons were negative.The A450 value of urine samples were determined by ELISA kit,based on which the S100 A6 protein content was calculated by using Curve Expert software.Logistic regression was performed by using SPSS 16.0 software,and ROC curve was plotted to evaluate the significance of S100 A6 in diagnosis of UTI.The urine samples in UTI group were stored at room temperature for 2,4,8,10,12,24,48 and 96 h,and analyzed for stability of S100 A6 protein content by Western blot.Results The urinary S100 A6 protein content in UTI group was significantly higher than that in normal control group(P<0.01).The Logistic regression equation was as follows:y=-0.971+0.04 x,with an OR value of 1.004,P=0.001.The area under ROC curve was 0.862,while the Youden index was 0.628.When the critical value of S100 A6 was set as 23.277 pg/mL,the sensitivity for diagnosis of UTI was 91.2%,while the specificity was 71.6%.The S100 A6 protein contents in urine after storage at room temperature for 2,4,8,10,12,24,48 and 96 h showed no significant change(P>0.05).Conclusion S100 A6 protein was significantly correlated to UTI,which was of a potential significance in diagnosis and might be used as a novel marker for primary screening of UTI.
