摘 要:目的:探讨使用磁力导航髓内钉内固定术治疗股骨干骨折对患者凝血功能及血清乳酸脱氢酶(LDH)、肌酸激酶(CK)、肌酸激酶同工酶(CKMB)水平的影响。方法:选取2017年2月至2020年2月期间云南省昭通市巧家县人民医院收治的41例股骨干骨折患者作为研究对象。根据随机数表法将这些患者分为对照组(n=20)和观察组(n=21)。为对照组患者使用传统的髓内钉内固定术进行治疗,为观察组患者使用磁力导航髓内钉内固定术进行治疗。然后观察两组患者术中的出血量、手术时长、术毕至可进行负重锻炼的时间、凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血活酶时间(APTT)、血清LDH的水平、血清CK的水平及血清CKMB的水平。结果:观察组患者术中的出血量少于对照组患者,其手术时长及术毕至可进行负重锻炼的时间均短于对照组患者,其术后24 h的PT、TT及APTT均长于对照组患者,其术后24 h血清LDH、CK及CKMB的水平均低于对照组患者,P<0.05。结论:使用磁力导航髓内钉内固定术治疗股骨干骨折对患者凝血功能的影响及对其造成的创伤小,引发的炎症反应轻微,其术后康复的速度快。Objective:To investigate the effects of magnetic navigated intramedullary nail fixation on coagulation function and serum levels of lactate dehydrogenase(LDH),creatine kinase(CK)and creatine kinase isoenzyme(CKMB)in the treatment of femoral shaft fracture.Methods:A total of 41 patients with femoral shaft fracture treated in Qiaojia County People’s Hospital of Zhaotong City,Yunnan Province from February 2017 to February 2020 were selected as the study subjects.The patients were divided into control group(n=20)and observation group(n=21)according to the random number table method.The control group was treated with traditional intramedullary nail fixation,and the observation group was treated with magnetic navigation intramedullary nail fixation.Then,intraoperative blood loss,duration of surgery,time to weight-bearing exercise after surgery,prothrombin time(PT),thrombin time(TT),activated partial thrombin time(APTT),serum LDH level,serum CK level and serum CKMB level were observed in 2 groups.Results:The intraoperative blood loss in the observation group was less than that in the control group,the duration of surgery and the time to carry out weight exercise after surgery were shorter than that in the control group,the PT,TT and APTT at 24 h after surgery were longer than that in the control group,the serum LDH,CK and CKMB levels at 24 h after surgery were lower than that in the control group,P<0.05.Conclusion:The use of magnetic navigation intramedullary nail internal fixation in the treatment of femoral shaft fracture has little influence on the coagulation function of patients,little trauma,little inflammatory reaction,and fast postoperative recovery.
