摘  要:目的:通过改变消融参数对猪肝脏和肌肉进行超声引导下激光消融,比较消融范围与消融时间和消融功率间的关系;比较不同组织消融范围与气化范围的差异;观察比较同一组织气化范围与消融范围的关系,以指导临床使用激光消融肿瘤。方法:超声引导下将激光光纤在21G穿刺针的引导下穿入组织前1/3处,退针约1 cm,在脉冲模式下以30 W、60 W、90 W的功率,对肝脏和肌肉分别消融30 s、60 s、90 s。消融结束后观察肝脏与肌肉的气化范围和消融范围的形态及大小。结果:其他条件相同时,在一定范围内,随着消融功率及消融时间的增加,肝脏和肌肉的消融范围与气化范围增加(P<0.05),当消融功率及消融时间达到一定程度时,其消融范围增加的趋势减慢,气化范围甚至减小;相同消融条件下,一定范围内,激光消融时肝脏的消融范围大于肌肉的消融范围;相同消融条件下,两种组织激光消融后的气化范围均与消融范围存在显著相关性(P<0.05)。结论:组织激光消融范围的大小与消融功率、消融时间及组织生物特性相关,使用超声监测到的消融后气化范围与消融范围存在显著相关性,可指导临床使用超声引导下激光消融。Objective:To compare the relationship between ablation range and ablation time and ablation power by ultrasonic guided laser ablation of liver and muscle in vitro by changing ablation parameters.To compare the difference between ablation range and gasification range of different tissues.To observe and compare the relationship between the same tissue gasification range and ablation range,so as to guide the clinical use of laser ablation of tumor.Methods:Under the guidance of ultrasound,the laser fiber was inserted into the tissue under the guidance of the puncture needle.The depth of the needle was about 1/3 of the length of the specimen,and the needle was withdrawn about 1 cm.Under the pulse mode,the power of 30 W,60 W and 90 W was used to ablate the liver and muscle for 30 s,60 s and 90 s respectively.After ablation,the shape and size of the gasification and ablation range of liver and muscle were observed.Results:When other conditions were the same,in a certain range,with the increase of ablation power and ablation time,the ablation range and gasification range of liver and muscle increased(P<0.05).When the ablation power and time reached a certain degree,the increasing trend of ablation range slowed down,while the gasification range even decreased.Under the same ablation conditions,the ablation range of liver was larger than that of muscle in a certain range.Under the same ablation conditions,the gasification range of liver and muscle after laser ablation was significantly correlated with the ablation range(P<0.05).Conclusion:The size of tissue ablation range is related to ablation power,ablation time and tissue biological characteristics.There is a significant correlation between gasification range and ablation range under ultrasound observation,which can guide the clinical use of ultrasound-guided laser ablation.
