摘 要:目的:探讨超声高低频探头联合扫查在女童性早熟卵巢发育情况中的应用价值。方法:选取2017年7月—2020年6月我院主诉为早期乳房增大的女童152例为研究对象,以促性腺激素释放激素(GnRH)激发试验为金标准,均采用超声高频探头、低频探头及高低频探头联合扫查,比较3种扫查方法的诊断价值。并根据高低频探头联合扫查结果将性早熟分为中枢性早熟(CPP)和单纯乳房发育(PT),比较两者的一般资料、血清激素(促黄体生成素(LH)、卵泡刺激素(FSH)、雌二醇(E2)、促甲状腺激素(TSH))水平及子宫、卵巢、乳腺参数测值,分析性早熟的超声图像特征。结果:GnRH激发试验显示152例主诉为早期乳房增大的女童中70例为性早熟,占比46.05%。超声高低频探头联合扫查诊断性早熟的敏感度(95.71%)、阴性预测值(95.89%)高于超声高频探头(80.00%、83.72%)、低频探头(71.43%、78.95%)单一扫查(P<0.05)。CPP患儿的身高、LH峰值、FSH峰值、LH/FSH峰值高于PT患儿(P<0.05)。CPP患儿的子宫长径、子宫前后径、子宫体积、卵巢长径、卵巢体积、最大卵泡直径、乳腺低回声区长径、乳腺低回声区厚度均大于PT患儿(P<0.05)。结论:超声高低频探头联合扫查可客观反映女童性早熟特征,间接反映GnRH的分泌情况,在真、假性性早熟的鉴别诊断中具有较大应用价值。Objective:To explore the application value of combined scanning of high and low frequency ultrasound probes in the development of female precocious ovaries.Methods:A total of 152 girls with early breast enlargement complaints from our hospital from July 2017 to June 2020 were selected as the research objects.The gonadotropin-releasing hormone(GnRH)provocation test was used as the gold standard,and the ultrasonic high-frequency probe,low-frequency probe,and high-lowfrequency probe were used for scanning.The diagnostic value of the three scanning methods was compared.And according to the results of the combined scanning of high-low-frequency probes,precocious puberty was divided into central precocious puberty(CPP)and premature thelarche(PT).The general information and serum hormone(luteinizing hormone(LH),follicle stimulating hormone(FSH),estradiol(E2),thyroid stimulating hormone(TSH))levels,the measured values of ultrasonic uterine,ovarian,and breast parameters of the two were compared,and the ultrasonic image characteristics of precocious puberty were analyzed.Results:The GnRH provocation test showed that 70 of 152 girls complained of early breast enlargement had precocious puberty,accounting for 46.05%.The sensitivity(95.71%)and negative predictive value(95.89%)of the combined scanning of ultrasound high-low-frequency probes were higher than that of single scanning of ultrasound high-frequency probes(80.00%,83.72%)and low-frequency probe(71.43%,78.95%)(P<0.05).The height,peak value of LH,peak value of FSH and peak value of LH/FSH of CPP were higher than those of PT(P<0.05).The long diameter of the uterus,the anteroposterior diameter of the uterus,the volume of the ovary,the diameter of the largest follicle,the long diameter of the hypoechoic area of the breast,and the thickness of the hypoechoic area of the breast in CPP patients were all greater than those in PT patients(P<0.05).Conclusion:The combined scanning of ultrasound high-low-frequency probes can objectively reflect the characteristics of precocious pu
