摘 要:目的采用心脏磁共振电影成像(cine-CMR)技术快速定量评价保留射血分数的肥厚型心肌病(HCM)患者的双心房结构及功能改变。方法回顾性分析2015年7月至2019年9月在本院诊断为HCM的患者资料,且左心室射血分数(LVEF)≥50%的患者53例,设为HCM组;同时选取同时期在本院行心脏磁共振(CMR)检查的53名健康体检者为对照组。测量HCM组及对照组双心房指标,包括左、右心房舒张末期及收缩末期纵径、横径及面积,并分别计算其纵径、横径及面积变化率。采用两独立样本t检验进行分析。结果 HCM组左心房舒张末期纵径、横径及面积均大于对照组,左心房横径、纵径及面积变化率均小于对照组,差异有统计学意义(P<0.05);HCM组右心房舒张末期纵径、横径、面积及纵径、横径、面积变化率与对照组比较,差异均无统计学意义(P>0.05)。结论采用cine-CMR技术能够快速评价保留射血分数的HCM患者双心房结构及功能的改变,在LVEF保留时期患者已有左心房结构及功能的改变。Objective Rapid quantitative assessment of the structural and functional changes of the biatrium in hypertrophic cardiomyopathy patients with preserved ejection fraction by cine cardiovascular magnetic resonance imaging(cine-CMR). Methods 53 patients who were diagnosised as hypertrophic cardiomyopathy with left ventricular ejection fraction(LVEF) ≥ 50% in our hospital from July 2015 to September 2019 were enrolled in this study, which was the HCM group. At the same time, 53 healthy patients undergoing cardiac magnetic resonance were selected as control group. The parameters of biatrium in HCM and control group were measured, including longitudinal diameter, transverse diameter and the area at the end of systolic and diastolic period, and then calculated the rate of changes.Independent-sample T tests were used to compare the various parameters between HCM and control group. Results The left atrium longitudinal diameter, transverse diameter and the area at the end of diastolic period in HCM group were significantly higher than that in control group(P<0.05). The change rates of left atrial longitudinal diameter, transverse diameter and area were less than that of the control group(P<0.05).The parameters of right atrium between the two groups made no significant difference(P>0.05). Conclusion Application of cine-CMR can rapidly assess the changes of structure and functionl of biatrium in hypertrophic cardiomyopathy patients with preserved ejection fraction.Changes of structure and function in the left atrium occur in these patients.
