摘 要:目的评价压缩感知(CS)电影成像技术在左心功能评价中的应用价值。方法前瞻性纳入148例患者,每例患者均采集传统短轴电影成像,自由呼吸下CS电影成像及单次屏气下CS电影成像,测量并比较左心室射血分数(LVEF),左心室心肌质量(LVM),通过配对样本t检验分析两种成像方式有、无统计学差异,采用线性回顾性分析及Bland-Alteman评价两种成像方法的一致性。结果将研究对象分成心律齐心功能正常组、心律齐心衰组、心律不齐心功能正常组、心律不齐心衰组。单次屏气下CS电影成像较传统电影成像相比,心律齐心功能正常组、心律齐心功能失代偿组,配对样本t检验均无统计学差异(P>0.1),线性回归相关性分析拟合度R2较高(0.685~0.863),Bland-Alteman相关性分析覆盖率均大于0.95;心律不齐心功能正常组线性回归分析拟合度R2较低(0.123、0.210),心律不齐心衰组心肌质量Bland-Alteman相关性分析覆盖率小于0.95(覆盖率为0.880)。自由呼吸下CS电影成像较传统电影成像相比,心律齐心衰组、心律不齐心衰组,配对样本t检验均无统计学差异(P>0.1),线性回归相关性分析拟合度R2较高(0.507~0.751),Bland-Alteman相关性分析覆盖率均大于0.95;心律齐心功能正常组配对样本t检验均有统计学差异(P<0.05),心律不齐心功能正常组线性回归相关性分析拟合度R2低(R2为0.040)、心肌质量Bland-Alteman相关性分析覆盖率小于0.95(覆盖率为0.913),所有研究对象LVEF配对样本t检验有统计学差异(P<0.001)。结论传统电影成像适用于心律齐研究对象左心功能评估,无论是自由呼吸下还是单次屏气下CS电影成像均适用于所有研究对象左心功能评价,CS电影成像应用范围较传统电影成像更加广泛。Objective This prospective study aimed to evaluate the diagnostic quality and accuracy of compressed sensing(CS) real-time cine cardiovascular magnetic resonance(CMR) in the assessment of left ventricular function. Methods 148 patients received a fully sampled segmented multiple BH cine sequence(standard of reference),a single BH and during free breathing(non-BH imaging) CS real time cine CMR. Quantitative analysis of LV function and mass was performed. Linear regression, Bland-Altmananalysis, and paired t testing were performed. Results There was no significant difference in the paired-samplet-test(P>0.1),linear regression correlation analysis showed that the fitness R2 was well(0.685-0.863),and the coverage of Bland-Altman correlation analysis was greater than 0.95 with the standard of reference for single-BH CS real time cine CMR for all of patients with rhythm. Linear regression correlation analysis showed that the fitness R2 was not good for arrhythmia(EF,0.123;mass, 0.210). The coverage of Bland-Altman correlation analysis was 0.880(<0.95). There was no statistical difference for paired-samplet-test in the heart failure group(P>0.1),linear regression correlation analysis showed that the fitness R2 was well(0.507-0.751),Bland-Altman correlation analysis coverage was greater than 0.95 for all of patients with heart failure with the standard of reference for free breathing CS real time cine CMR.There was statistical difference for paired-samplet-test in the normal heart function and rhythm group(P<0.05). Linearregression correlation analysis showed that the fitness R2 was not good for EF(0.040),the coverage of Bland-Altman correlation analysis was 0.913(<0.95) for mass in the normal heart function and arrhythmia group. Conclusion Traditional cine imaging is suitable for left ventricular function evaluation of arrhythmia research subjects, and compressive sensing film imaging is applicable to all subjects’ left ventricular function evaluation.
