摘  要:目的应用多层螺旋CT(MSCT)探讨胸骨中线孔(胸骨孔)特征及其毗邻组织,为临床避免胸骨介入潜在危险因素提供安全范围。方法回顾性分析2018年1月至12月2000例胸部MSCT资料。通过观察轴位、冠状位和矢状位的多平面重组(MPR)和容积再现(VR)图像,分析胸骨孔一般特征及其毗邻结构,并对胸骨孔宽度、皮下脂肪厚度、胸骨孔与毗邻组织距离和皮肤到心包的距离进行测量。结果胸骨孔发生率为4.6%(92/2000),74例(80.4%)胸骨孔位于胸骨下段第五肋软骨交界处(CCJ),12例(13.1%)位于第六CCJ,6例(6.5%)位于第四CCJ。胸骨孔后方毗邻结构为肺实质42例(45.6%),其次是纵隔脂肪33例(35.9%),心包17例(18.5%)。胸骨孔宽度为(4.85±2.79)mm,相应皮下脂肪厚度为(9.93±5.47)mm,皮肤至心包的距离为(29.93±8.78)mm,胸骨孔与毗邻组织的距离为(4.16±6.07)mm。结论胸骨孔常在第四至第六CCJ,常毗邻心肺组织,胸骨介入治疗应避免在该区域进行干预。Objective To explore the characteristics of the midline sternal foramen(sternal foramen) and its adjacent tissues by using multi-slice spiral CT(MSCT),so as to provide a safe range for avoiding the potential risk factors of sternal intervention. Methods The data of 2000 chest MSCT cases from January to December 2018 were retrospectively analyzed.Through the observation of MPR and VRT images in axial, coronal and sagittal position, the general characteristics of the sternum foramen and its adjacent structures were analyzed, and the width, subcutaneousthickness, distance between the sternum foramen and adjacent tissues, and the distance from skin to pericardium were measured. Results The incidence of sternal foramen was 4.6%(92/2000).The foramina of sternum was located at the junction of the 5 th costal cartilage of the lower sternum(CCJ) in 74 cases(80.4%),the 6 th CCJ in 12 cases(13.1%),and the 4 th CCJ in 6 cases(6.5%).Lung parenchyma was found in 42 cases(45.6%).Mediastinal fat was followed in 33 cases(35.9%) and pericardium in 17 cases(18.5%).The mean width of the sternal foramen was(4.85±2.79)mm, the corresponding subcutaneous fat thickness was(9.93±5.47)mm, and the mean distance from skin to pericardium was(29.93±8.78)mm.The distance between the sternum foramen and the adjacent tissue was(4.16±6.07)mm. Conclusion The sternal foramen is often located in the 4 th to 6 th CCJ,which is adjacent to the cardiopulmonary tissue. Intervention should be avoided in this area.
