摘 要:目的探讨剂量指数(DRI)结合头围在降低儿童头颅CT检查辐射剂量的可行性。方法将行头颅CT检查的300例患儿依据其头围进行分组,头围≤450 mm为A组,450 mm﹤头围≤500 mm为B组,头围﹥500 mm为C组;三组患儿分别采用随机数表法再分为若干亚组,各亚组分别依次选取27、26、25、24、23、22等6个DRI进行扫描,扫描管电压100 kV,DoseRight技术,采用iDose4重组算法进行图像重组,然后对图像质量进行主观及客观评价,并对辐射剂量进行对比分析。结果在其他扫描条件相同的情况下,A组患儿DRI的最小值为23,B组患儿DRI的最小值为24,C组患儿DRI的最小值为25;三组患儿图像在基底节区及半卵圆中心区信噪比(SNR)均随着DRI的增加而增加,差异有统计学意义(P﹤0.01);三组患儿的CT容积剂量指数(CTDIvol)和体型特异性剂量评估值(SSDE)均随DRI的减小而降低,差异有统计学意义(P﹤0.01)。结论 DRI结合头围能够对儿童头颅CT扫描个性化扫描方案的制定提供参考依据,能够在保证图像质量的前提下降低辐射剂量。Objective To explore the feasibility of DoseRight index combined with head circumference in reducing the radiation dose of skull CT examination in children. Methods According to the head circumference, 300 children who underwent CT examination were divided into groups, group A with head circumference≤450 mm, group B with 450 mm﹤head circumference≤500 mm, group C with head circumference﹥500 mm, children in the three groups were regrouped with 20 cases as a small group, six DRIs(27,26,25,24,23,22) were selected for scanning,the tube voltage was 100 kV and the DoseRight technology was used.iDose4 reconstruction algorithm was used for image reconstruction, then the image quality was evaluated subjectively and objectively, and the radiation doses were compared. Results Under the same scanning conditions, the smallest value of DRI in group A was 23,in group B was 24,and in group C was 25;the SNR in basal ganglia and centrum semiovale increased with the increase of DRI,and the difference was statistically significant(P<0.01). TheCTDIvol and SSDE of the three groups decreased with the decrease of DRI(P<0.01). Conclusion The combination of DoseRight Index and head circumference can provide reference for making personalized scanning scheme of children and reduce radiation dose while ensuring image quality.
