摘  要:目的观察慢性肾衰竭患者血清铁蛋白(serum ferritin,SF)、全段甲状旁腺激素(intact parathyroid hormone,iPTH)水平,探讨其与血脂代谢的关系。方法113例慢性肾衰竭患者,根据肾功能分级分为代偿期组34例,失代偿期组26例,肾功能衰竭期组29例,尿毒症终末期组24例;选择同期体检健康者30例为对照组。应用全自动生化分析仪检测血肌酐(serum creatinine,SCr)、三酰甘油(three acyl glycerin,TG)、总胆固醇(total cholesterol,TC)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)水平;采用电化学发光免疫法检测SF水平,采用ELISA法检测iPTH水平,并进行各组间比较。采用Pearson相关性分析慢性肾衰竭患者血脂各参数、SF、iPTH与SCr的相关性以及血清SF、iPTH水平与血脂的相关性。结果慢性肾衰竭各组患者血清TG水平均高于对照组(P<0.05);代偿期组、失代偿期组、尿毒症终末期组血清TC水平均高于对照组(P<0.05),失代偿期组、肾功能衰竭期组血清LDL-C水平均高于对照组(P<0.05),尿毒症终末期组血清HDL-C水平均低于对照组、代偿期组和肾功能衰竭期组(P<0.05)。对照组血清SCr[(60.82±14.03)μmol/L]、SF[(137.56±21.58)μg/L]和iPTH水平[(59.28±13.58)ng/L]均低于代偿期组[(152.32±35.20)μmol/L、(189.56±41.25)μg/L、(76.82±22.14)ng/L]、失代偿期组[(296.25±67.82)μmol/L、(277.65±63.58)μg/L、(95.62±31.25)ng/L]、肾功能衰竭期组[(589.37±112.05)μmol/L、(391.52±85.81)μg/L、(173.58±52.25)ng/L]、尿毒症终末期组[(897.85±181.05)μmol/L、(564.82±125.63)μg/L、(428.56±152.32)ng/L](P<0.05),代偿期组低于失代偿期组、肾功能衰竭期组和尿毒症终末期组(P<0.05),失代偿期组低于肾功能衰竭期组和尿毒症终末期组(P<0.05),肾功能衰竭期组低于尿毒症终末期组(P<0.05)。慢性肾衰竭患者血清TG、TC、LDL-C、SF、iPTH与SCr均呈正相关Objective To observe the levels of serum ferritin(SF)and intact parathyroid hormone(iPTH)in patients with chronic renal failure,and to explore their relationships with blood lipid metabolism.Methods According to renal function classification,113 patients with chronic renal failure were divided into compensatory stage group(n=34),decompensatory stage group(n=26),renal failure stage group(n=29)and end-stage uremia group(n=24).Another 30 healthy volunteers were as controls(control group).The levels of serum creatinine(SCr),triacylglycerol(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C)and high-density lipoprotein cholesterol(HDL-C)were detected by automatic biochemical analyzer.Electro-chemiluminescence immunoassay was used to detect SF level and ELISA was used to detect iPTH level,and the comparison was made among groups.Pearson correlation was used to analyze the correlations of serum lipid parameters,SF and iPTH with SCr,as well as the correlations of serum SF and iPTH levels with blood lipid.Results The TG level was higher in each group of chronic renal failure patients than that in control group(P<0.05).The level of TC was higher in compensatory stage group,decompensatory stage group and end-stage uremia group than that in control group(P<0.05).The level of LDL-C was higher in decompensatory stage group and renal failure stage group than that in control group(P<0.05).The level of HDL-C was lower in end-stage uremia group than that in control group,compensatory stage group and renal failure stage group(P<0.05).The levels of SCr,SF and iPTH were lower in control group((60.82±14.03)μmol/L,(137.56±21.58)μg/L,(59.28±13.58)ng/L)than those in compensatory stage group((152.32±35.20)μmol/L,(189.56±41.25)μg/L,(76.82±22.14)ng/L),decompensatory stage group((296.25±67.82)μmol/L,(277.65±63.58)μg/L,(95.62±31.25)ng/L),renal failure stage group((589.37±112.05)μmol/L,(391.52±85.81)μg/L,(173.58±52.25)ng/L),and end-stage uremia group((897.85±181.05)μmol/L,(564.82±125.63)μg/L,(428.56±15
