鼻内窥镜协助下等离子刀治疗儿童鼻出血的疗效观察

摘　　要：目的探究鼻内窥镜协助下等离子刀治疗儿童鼻出血的疗效。方法选取2019年1—12月本院收治的120例门诊鼻中隔黎氏区鼻出血患儿作为研究对象,根据单双号随机分为观察组和对照组,各60例。对照组采用传统鼻腔填塞术治疗,观察组采用鼻内窥镜协助下等离子刀治疗,比较两组患儿临床疗效、并发症发生率、鼻腔总出血量(以纱布、止血棉球、棉片以及吸引器瓶中重量预估出血量)、鼻腔黏膜恢复时间、通气恢复时间、愈合时间以及随访3个月后复发率,并在治疗前、治疗中、治疗后填塞期评估疼痛评分。结果两组并发症发生率比较差异无统计学意义。观察组各时间段VAS评分、随访3个月后复发率均低于对照组,治疗总有效率高于对照组,鼻腔黏膜恢复、通气恢复时间、愈合时间均长于对照组,鼻腔总出血量少于对照组(P<0.05)。结论对鼻中隔黎氏区鼻出血患儿实施鼻内窥镜协助下等离子刀治疗效果显著,但会延长鼻腔黏膜恢复时间。Objective To explore the effect of plasma knife assisted by nasal endoscope in the treatment of epistaxis in children.Methods 120 children with epistaxis in the Lai's area of the nasal septum in the outpatient department from January 2019 to December 2019 were selected as the research subjects.According to the odd and even numbers,they were randomly divided into observation group and control group,with 60 cases in each group.The control group was treated with traditional nasal cavity tamponade,and the observation group was treated with plasma knife assisted by nasal endoscope.The clinical efficacy,incidence of complications,total nasal blood loss(estimated by weight of gauze,tampon,cotton pad and aspirator bottle),nasal mucosa recovery time,ventilation recovery time,healing time and recurrence rate after 3 months of follow-up were compared between the two groups,and pain score was assessed before,during and after treatment.Results The VAS score at each time and the recurrence rate after 3 months of follow-up in the observation group were lower than those in the control group,the total effective rate was higher than that in the control group,the nasal mucosa recovery time,ventilation recovery time and healing time were longer than those in the control group,and the total nasal bleeding was lower than that in the control group(P<0.05).Conclusion Plasma scalpel assisted by nasal endoscope is effective in the treatment of children with nasal septum nosebleed,but it can prolong the recovery time of nasal mucosa.
