摘 要:目的:探讨呼吸重症监护室(Respiratory Intensive Care Unit,RICU)护士在为机械通气患者进行拍背操作时所致疼痛现况,并分析引发患者疼痛的影响因素,简要探讨对患者的睡眠影响。方法:选取2019年1月至2019年12月山东大学齐鲁医院收治的术后机械通气患者98例作为研究对象,采用中文版行为疼痛评估量表、Richmond躁动-镇静评分量表进行调查。结果:拍背操作可以明显增加患者的疼痛感,最高疼痛评分为11分,有疼痛感受的患者有97例,占98.98%,拍背操作后5 min患者的疼痛强度恢复到拍背前5 min水平;相关分析结果显示,患者的疼痛强度与RASS评分、痰液黏稠度、既往手术史相关。结论:拍背操作可以明显增加RICU机械通气患者术后24 h内的疼痛强度,需要在临床中引起重视。Objective:To explore the current situation of pain caused by back slapping nursing for patients with mechanical ventilation in Respiratory Intensive Care Unit(RICU),and to analyze the influencing factors of patients′pain.Methods:A total of 98 patients with postoperative mechanical ventilation treated in Qilu Hospital of Shandong University from January 2019 to December 2019 were selected as the research objects.The Chinese version of Pain Behavior Scale and Richmond Agitation Sedation Scale(RASS)were used for investigation.Results:Back slapping can significantly increase the pain of patients.The highest pain score was 11.There were 97 patients with pain feeling,accounting for 98.98%.The pain intensity of patients recovered to the level of 5 minutes before back slapping 5 minutes after back slapping.The results of correlation analysis showed that the pain intensity was related to RASS score,sputum viscosity and previous operation history.Conclusion:Back slapping can significantly increase the pain intensity within 24 hours after mechanical ventilation in RICU,which needs attention in clinical application.
