不同首次换药时间对清创缝合术后患者伤口愈合、疼痛的影响比较

摘　　要：目的分析不同首次换药时间对清创缝合术后患者伤口愈合与疼痛的影响。方法将156例行清创缝合术患者随机分为对照组与观察组,每组各78例。对照组患者在手术后2~3 d开展首次换药,观察组患者在手术后1 d给予首次换药,分别将两组患者的伤口愈合情况、伤口疼痛程度以及伤口局部不良反应进行统计学比较。结果观察组患者的伤口愈合效果明显好于对照组,伤口愈合时间明显短于对照组,术后3 d、5 d与7 d的VAS评分明显低于对照组,并且伤口局部不良反应总发生率显著低于对照组,差异均具有统计学意义(P<0.05)。结论在清创缝合术后1 d开展首次换药不仅能够更好地保障伤口愈合效果与愈合速度,也对术后疼痛有明显改善作用,并且也能积极抑制伤口局部不良反应的发生概率。Objective To analyze the effect of different first dressing change time on wound healing and pain of patients after debridement and suture.Methods One hundred and fifty-six patients undergoing debridement and suture were randomly divided into control group and observation group,with 78 cases in each group.Patients in control group changed their dressing for the first time 2-3 days after operation,while patients in observation group were given the first dressing change one day after operation.The wound healing,wound pain and local adverse reactions of the two groups were statistically compared.Results The effect of wound healing in the observation group was significantly better than that in the control group,and the wound healing time was significantly shorter than that in the control group.The VAS scores on the 3th,5th and 7th postoperative days in the observation group were significantly lower than those in the control group,and the total incidence of local adverse reactions in the wound was significantly lower than that in the control group(P<0.05).Conclusion The first dressing change one day after debridement and suture can not only better ensure the healing effect and speed of wound,but also significantly improve the postoperative pain,and can also actively inhibit the incidence of local adverse reactions.
