儿童紫癜性肾炎血尿NGAL、尿RBP的检验结果及不同剂量雷公藤多苷片的效果研究

摘　　要：目的探讨儿童紫癜性肾炎(HSPN)血尿NGAL、尿RBP检测的价值及不同剂量雷公藤多苷片的治疗效果。方法选入72例HSPN患儿作为观察组,均予以雷公藤多苷片口服治疗,根据用药量不同分为小剂量组[1 mg/(kg•d)]和大剂量组[1.5 mg/(kg•d)],每组36例;并选入无肾脏损伤的HSP患儿50例作为对照组;选入同期健康体检儿童50名作为正常组。检测各组血尿NGAL、尿RBP,评价治疗效果。结果入组时,观察组血尿NGAL、尿RBP含量均显著高于正常组和对照组(P<0.05);经雷公藤多苷片治疗后,HSPN患儿血尿NGAL、尿RBP含量均较治疗前明显下降(P<0.05),而大剂量组下降幅度更大,与小剂量组差异显著(P<0.05),且大剂量组紫癜消退时间、蛋白尿及潜血转阴时间亦明显低于小剂量组(P<0.05),愈显率明显高于小剂量组(P<0.05)。结论血尿NGAL、尿RBP检测可作为HSPN早期诊断及预后评估的依据;雷公藤多苷片治疗HSPN疗效明显,且大剂量更具优势。Objective To investigate the value of hematuria NGAL and urine RBP in children with purpura nephritis(HSPN)and the therapeutic effect of different doses of tripterygium wilfordii polyglycoside tablets.Methods A total of 72 children with HSPN were selected as the observation group and given tripterygium wilfordii polyglycoside tablets.According to different dosage,they were divided into small dose group[1 mg/(kg•d)]and large dose group[1.5 mg/(kg•d)],36 patients in each group.Fifty children with HSP without renal injury were selected as the control group.50 healthy children were selected as normal group.Hematuria NGAL and urine RBP were detected in each group.Evaluate the effect of treatment.Results The hematuria NGAL and urine RBP contents in the observation group were significantly higher than those in the normal group and the control group(P<0.05).After treatment with tripterygium wilfordii glucoside piece more,children with HSPN hematuria NGAL,urine RBP content than before treatment significantly decreased(P<0.05),and high-dose group decreased more,and small dose group significant difference(P<0.05),and high-dose group purpura regression time,proteinuria and occult blood turns shade also significantly lower than the low-dose group(P<0.05),markedly effective rate was obviously higher than low-dose group(P<0.05).Conclusion Hematuria NGAL and urine RBP can be used as the basis for early diagnosis and prognosis evaluation of HSPN.Tripterygium wilfordii polyglycoside tablet has obvious curative effect in the treatment of HSPN.
