双腔中心导管穿刺置管治疗心包积液在急诊中的应用

摘　　要：目的研究在急诊中应用双腔中心静脉导管穿刺置管术诊疗心包积液的效果。方法选择中到大量心包积液患者32例,使用双腔中心静脉导管经皮穿刺心包腔并留置中心静脉导管,对心包积液及时和反复抽吸或引流,并及时行心包腔冲洗和药物治疗,取得了良好的诊断和治疗效果。结果32例手术均一次成功,手术时间10~30 min,引流量400~6000 mL,中心静脉导管留置时间4~31 d。结论在急诊急救中使用双腔中心静脉导管置管术治疗心包积液,简单方便效果显著,且损伤程度小,并发症少。还可以考虑应用到胸膜腔腹膜腔等其他浆膜腔积液诊疗中。Objective To study the effect of double lumen central venous catheter in the diagnosis and treatment of pericardial effusion in emergency.Methods A total of 32 patients with pericardial effusion were enrolled in this study.We used double lumen central venous catheter to puncture pericardial cavity and indwelling central venous catheter,timely and repeatedly aspiration or drainage of pericardial effusion,and timely pericardial cavity flushing and drug treatment had achieved good diagnosis and treatment effect,the diagnosis and cure rate had been significantly improved and the life of patients had been prolonged.Results The operation was successful in all 32 cases.The operation time was 10~30 minutes,the drainage volume was 400~6000 ml,and the indwelling time of central venous catheter was 4~31 days.Conclusion The application of double lumen central venous catheter in the emergency treatment of pericardial effusion is simple,convenient and effective,with less damage and fewer complications.It can also be used in the diagnosis and treatment of pleural cavity,peritoneal cavity and other serous cavity effusion.
