摘 要:目的研究雷公藤多苷抑制miR-146a表达对外周血中Th17和Treg细胞构成比进行调控的作用机制。方法选取60例类风湿关节炎(RA)患者,然后将其随机分为雷公藤组和安慰剂组,召集同时间的60例健康志愿者作对照组。检测三组外周血中miR-146a表达水平、Th17细胞构成比、Treg细胞构成比。在此之后雷公藤组以1 mg/kg,每日3次服用6周雷公藤多苷片,安慰剂组按相同剂量服用6周安慰剂后再次检测外周血中miR-146a表达水平、Th17细胞构成比和Treg细胞构成比。结果RA组患者miR-146a、Th17细胞构成比高于对照组,而Treg细胞构成比则低于对照组,差异均有统计学意义(t分别=13.62、26.39、29.93,P均<0.05)。雷公藤组服药6周后,miR-146a、Th17细胞构成比较服药前有所下降,Treg细胞构成比较服药前有所上升,差异均有统计学意义(t分别=6.77、37.74、13.48,P均<0.05)。安慰剂组服药6周后的改变幅度并不明显,差异无统计学意义(t分别=1.12、0.12、0.03,P均>0.05)。雷公藤组服药6周后,miR-146a、Th17细胞构成与对照组比较,差异均无统计学意义(t分别=0.91、0.39,P均>0.05),Treg细胞构成比较对照组较低,差异有统计学意义(t=2.34,P<0.05)。结论雷公藤多苷能够通过下调miR-146a表达水平从而减少Th17细胞构成比,增加Treg细胞构成比,进而缓解RA患者的病情。Objective To explore the mechanism of Tripterygium wilfordii polyglycosides inhibiting the expression of miR-146a and regulating the composition ratio of Th17 and Treg cells in peripheral blood.Methods Sixty patients with rheumatoid arthritis were selected,and then randomly divided into the tripterygium wilfordii group and the placebo group.Sixty healthy volunteers were recruited at the same time for control group,and then the next experiment was conducted.The expression level of miR-146a,Th17 cell composition ratio and Treg cell composition in peripheral blood of each group were detected.After that,the Tripterygium wilfordii group was given 1 mg/kg Tripterygium wilfordii polyglycoside tablets,three times a day for 6 weeks.The placebo group was given the same dose of placebo for 6 weeks.After that,the expression level of miR-146a,Th17 cell composition ratio and Treg cell composition ratio in peripheral blood were per-formed again.Results The expression level of miR-146a and Th17 cell composition in peripheral blood of RA patients were significantly higher than those of the control group,while the proportion of Treg cells was lower than of the control group(t=13.62,26.39,29.93,P<0.05).After 6 weeks of drugs treatment,the expression level of miR-146a and the co-mposition ratio of Th17 cells of Tripterygium wilfordii group were significantly lower than before administra-tion,the composition of Treg cells was significantly higher than before(t=6.77,37.74,13.48,P<0.05).In placebo group,the expression level of miR-146a,the composition ratio of Th17 cells and Treg cells after treatment were not statistically different(t=1.12,0.12,0.03,P>0.05).After 6 weeks of drugs treatment,the expression level of miR-146a and the composition ratio of Th17 cells of Tripterygium wilfordii group were not statistically different from the control group(t=0.91,0.39,P>0.05),while the composition ratio of Treg cells was lower than that of the control group(t=2.34,P<0.05).Conclusion Tripte-rygium tripterygii can reduce the proportion of Th17 c
